SANTRAL YERLEŞİMLİ AKCİĞER KANSERİ TANISINDA BRONKOSKOPİNİN ROLÜ

Özet
Amaç: Santral yerleşimli akciğer kanseri vakalarında fiberoptik bronkoskopinin ( FOB) farklı yöntemlerinin tanısal değerlerini değerlendirmeyi amaçladık. Gereç ve Yöntem: Santral yerleşimli akciğer kanserinin tanısında fiberoptik bronkoskop (FOB) ile farklı prosedürler kullanılmaktadır. Çalışmamıza santral yerleşimli akciğer kanseri saptanan 218 art arda olgu (2 kadın, 216 erkek, ortalama yaş: 60 ± 10, 19 ile 83 yaş arasında) retrospektif olarak taranarak alınmıştır. Bulgular: Endobronşial lezyondan yapı- lan bronş biyopsisi (BB) 182 olguda tanısal (182/194; %94) sonuç verirken, iğne aspirasyonu ile elde edilen sitolojik materyal ise 96 olguda tanısal (96/123; %78) sonuç vermiş- tir. Endobronşial lezyonun örneklenmesinde BB ve TBİA birlikte kullanıldığında tanı ba- şarısı %95 olarak bulunmuştur. Wang-122 iğnesi kullanılarak yapılan TBİA ile lenf nodu örneklemesi 12 olguda tanısal sonuç vermiş ve 7 olguda tek tanı yöntemi olarak bulunmuştur. Çalışmamızda BB+firçalama+ tümör veya lenf nodundan TBİA yöntemleri uygulandığında FOB'un tanı değeri (%97) oldukça yüksek olarak bulunmuştur. Sonuç: Lenf nodu veya endobronşial lezyonun TBİA ile örneklenmesi ve fırçalama metodları santral yerleşimli akciğer kanserinin tanısında BB ile birlikte göz önüne alınmalıdır.
Abstract
Objective: We aimed to evaluate the diagnostic accuracy of different FOB procedures in centrally located lung cancer.Materials and methods: Different procedures using fiberoptic bronchoscopy have been used for diagnosis of centrally located lung cancer. 218 centrally located lung cancer cases (2 female, 216 male, mean age: 60 ± 10, with range of 19 to 83 years) were consequtively included in the study and retrospectively analyzed.Results: Forceps biopsy (FB) of endobronchial lesion gave a diagnostic result in 182 pts (182/194; 94%), cytology by using needle aspiration was diagnostic in 96 pts (96/123; 78%). Diagnostic success was 95% when we used FB+TBNA together to take samples from endobronchial lesions. TBNA by using Wang-122 needle from the mediastinal lymph node was diagnostic in 12 cases and in 7 cases it was the only diagnostic method. We have an impressive diagnostic success (97%) of the FOB in this group of patients when we used FB+brushing+TBNA from the tumour or lymph nodesConclusion: It was concluded that, brushing and TBNA from the tumour or lymph node should be considered in centrally located lung cancer diagnosis in addition to the FB
