Özet

Hiperhidroz vücudun normal fizyolojik ihtiyacından daha fazla ter salgılanması ile karakterize bir hastalıktır ve primer veya sekonder olarak 2 gruba ayrılabilir. Sekonder hiperhidroz altta yatan sistemik bir hastalığın göstergesi iken, primer hiperhidrozun nedeni belli değildir ve çoğunlukla avuç içi, ayak tabanı ve aksiler bölgeyi etkiler. Günümüzde primer hiperhidroz tedavisi başlıca lokal antiperspiranlar ve iyontoforez tedavisini içermektedir.İyontoforez, elektrik akımı kullanılması yoluyla deri içine çeşitli iyonların girişidir ve hiperhidroz tedavisindeki etkinliği ve güvenilirliği uzun yıllardır bilinmektedir. Bu nedenle lokal tedavilere cevap alınamayan durumlarda invaziv girişimden önce iyontoforez iyi bir tedavi seçeneği olarak kullanılabilir. Ancak hastaların semptomsuz kalabilmeleri için idame tedavisi gereklidir. Musluk suyu ile uygulanabildiği gibi antikoliner-jik ajanlar ve botulinum toksini de ilave edilebilir. Farklı uygulama önerileri olsa da, genellikle haftada 3-5 seans uygulanır. Bu makalede, hiperhidroz tedavisinde uygulanan iyonto-forezin klasik ve güncel uygulama yöntemlerini gözden geçirerek literatür bilgileri ışığında derlemeyi amaçladık.

Abstract

Hyperhidrosis is a disease characterized by the secretion of sweat in excess to the normal physiologic needs of the body and can be classified in 2 groups as a primary or secondary hyperhidrosis. While secondary hyperhidrosis is an indicative of an underlying systemic disease, the cause of primary hyperhidrosis is unknown and most commonly affects palms, soles and axillae.Current primary hyperhidrosis therapy mainly includes local antiperspirants and iontophoresis. Iontophoresis is the entry of various ions into the skin by using electric flow and its effectiveness and reliability in the treatment of hyperhidrosis have been known for years. Therefore in the cases that no reply taken to treatments, iontophoresis can be used as a good treatment choice before the invasive initiative is planned. However, to keep the patients symptom free, maintanence treatments are required. Although it can be applied with tap water, anticholinergic agents and botulinum toxin can be added. Even if there may be different application advices, 3 or 5 sessions per week is especially advised. In this artcile, we aimed to review the classic and current therapeutic strategies for iontophoresis to treat hyperhidrosis in the light of data in literature.
