Özet

Amaç: Son 3 yıl içinde serebrovasküler olay (SVO) geçirenhemipleji sekelli hastalarda fonksiyonel durumu ile, uyku kali-tesi ve depresyon arasındaki ilişkinin incelenmesidir.Gereç ve Yöntem: Çalışmaya yaşları 38-85 arasında olan 42 hemiplejik hasta alındı. Hastalar muayene edilerek her bir ekstremitenin Modifiye Ashworth Skalasına göre spastisitesi ve üst ve alt ekstremite ve el Brunstromme değerleri kaydedildi. Hastaların fonksiyonel durumları Fonksiyonel Bağımsızlık Ölçütü (FBÖ) ile, uyku kaliteleri Pittsburgh Uyku Kalitesi indeksi (PUKİ) ile, duygu durumları ise Beck Depresyon Ölçeği ile değerlendirildi. Bulgular: Hastaların yaş ortalaması 64,9±11,6 olarak hesap- landı. Çalışmaya alınan hastaların 20’si (% 47,6) erkek 22’si (% 52,4) kadındı. Ortalama hemipleji süreleri 6,9±8,5 ay ola- rak hesaplandı. Hastaların 40'ı (% 95,2) iskemik, 2'si (% 4,8) hemorajik SVO geçirmiş. Hastaların üst ekstremite Brunstromme ortalamaları 2,43±1,55, alt ekstremite Brunstromme ortalama- ları 3,45±1,78, el Brunstromme ortalamaları 2,09±1,57 idi. Spastisiteleri Modifiye Ashworth Skalasına göre üst ve alt eks- tremitede global olarak değerlendirildiğinde, üst ekstremite spastisite ortalamaları 1,21±1,16, alt ekstremite spastisite ortalamaları 0,79±1,12 olarak bulundu. Hastaların FBÖ puan ortalamaları 72,90±29,40, Pittsburgh Uyku Kalitesi İndeksi puan ortalamaları 8,48±4,80, Beck Depresyon Ölçeği puan ortalamaları 15,19±9,12 olarak hesaplandı. Abstract

Aim: The aim of this study is to analyse the relation between functional status, sleep quality and depression in hemiplegic patients who had experienced serebrovascular disease (SVD) in last three years. Materials and Methods: 38-85 year-old 42 patients were inc- luded in the study. All of the patients were examined and sco- res of each extremity’s spasticity according to Modified Ashworth Scale (MAS) and upper and lower limb and hand Brunstromme scores were noted. Patient’s functional status was assessed with Functional Independence Measure (FIM), sleep qualities with Pittsburgh Sleep Quality Index (PSQI) and moods with Beck Depression Scale, respectively. Results: The average age was 64.9±11.6 years. Twenty pati- ents (% 47,6) were male, and 22 of them (% 52,4) were fema- le. Mean duration of hemiplegia in patients was 6,9±8,5 months. 40 patients (% 95,2) had ischemic SVD, 2 of them hemorrhagic SVD. The patient’s mean upper, lower limb, and hand Brunstromme scores were 2.43±1.55, and 3.45±1.78, and 2.09±1.57, respectively.
