Hipofizer yetmezlik ve Hashimoto tiroiditinin birlikte görüldüğü bir olgu

Özet

Hipopituitarizm ön hipofiz bezinde üretilen bir veya birden fazla hormonun ya da tüm hormonların yetersizliği sonucu gelişen klinik sendromlara verilen isimdir. Hipopituitarizm intrinsik veya primer hipofiz hastalığı, intrinsik hipotalamik veya sekonder hipofiz hastalığı ve ekstrinsik ekstrasellar veya parasellar hastalığa bağlı oluşabilir. Hipofizer yetmezlikde TSH düzeyi düşük gözlenir. Fakat hastamızda TSH düzeyi yüksek bulundu. Yapılan ek tetkikler sonucunda beraberinde hashimoto tiroiditi saptadık. Hipofizde hemorajik sekeli olan, yalnız TSH yüksekliği (hashimoto tiroiditi saptanan) nedeniyle parsiyel hipopitüitarizm tanısı alan olguyu ender görülmesi nedeniyle sunuyoruz.

Abstract

Hypopituitarism is the clinical syndrome which results fromfailure of the anterior pituitary gland to produce one, morethan one or all of its hormones. Hypopituitarism can resultfrom intrinsic or primary pituitary disease, intrinsic hypothalamicor secondary pituitary disease, and extrinsic extrasellaror parasellar disease. Low levels of TSH are observed in pituitarydeficiencies, but TSH levels were higher in our patient.We diagnosed concomitant Hashimoto thyroiditis as a result offurther evaluation. We present this case which is very rarelycited in the literature.
