Tip 1 kapitellum kırıklarında cerrahi sonuçlarımız

Özet
Amaç: Kapalı tip 1 kapitellum kırıklarında akut rack vida ile açık redüksiyon internal fiksasyon ile tedavi sonuçlarımızı değerlendirmek istedik.Gereç ve Yöntem: Çalışmaya kapalı kapitellum kırığı nedeni ile cerrahi uyguladığımız 13 hasta alındı. Sekizi erkek, 5'i kadındı. Hastalarımızın yaşları ortalama 18,5'ti. Hastalarımızın tamamı tip 1 kırık. Hastalarımızdan birinde kapitellum kırığı ile beraber ipsilateral subtrokanterik femur şaft kırığı diğerinde ipsilateral kollum femoris kırığı mevcuttu. Bir hastamız bilateral kapitellum kırığı idi. Bilateral kapitellum kırığı olan hastamızın sağ dirsekte lateral kondil kırığıda mevcuttu. Hastalarımızın hepsine kapitellum kırığına yönelik acut track vida kullanıldı. Radyolojik değerlendirmelere Mayo Clinic Performance Index fort he Elbow (MCPIE) de eklendi. Hastalar ortalama 26 ay (6 ay-48 ay) takip edildi.Bulgular: MCPIE sonuçlarımız % 99.1 kusursuz, % 0.1 iyi olarak bulundu. Hastalarımızın yalnızca birinde rezidül agrı sıkıntısı oldu. Hastalarımızın hiçbirinde artrit, avasküler nekroz, infeksiyon, heterofik ossifikasyon, eklem hareket açıklığı kaybı olmadı.Sonuçlar: Kapitellum kırıklarında dirsek biyomekaniğini sağlamak için anatomik redüksiyon sağlamak zorunluluğu vardır.
Abstract
Backraund: We wanted to evaluate the clinical results aboutthe closed type 1 capitellum fractures with open reduction andinternal fixation of acute rack screws.Material and Methods: Our study population consisted of 13patients (8 men, and 5 women) with a median age of 18.5 years(range, 12-25 years) on whom we performed surgery with theindication of closed capitelluım fracture. All of our patients withtype 1 capitellum fracture. In one of our patients capitellumfracture was associated with ipsilateral subtrochanteric femoralshaft fracture, and in the other patient with ipsilateral collumfemoris fracture. One of our patients had bilateral capitellumfractures. The patient with bilateral fractures also had capitellumfracture of the lateral condyle of his right elbow. Acutetrack screws were used in all patients for capitellum fractures.Mayo Clinic Performance Index for the Elbow (MCPIE) wasalso added to the radiological examinations. Patients were alsofollowed for an average of 26 months (6 months -48 months).Results: According to MCPIE criteria, our results were excellent, and good in 99. 1, and 0.1 % of the patients respectively.Only one patient had resudial pain. None of our patients hadarthritis, avascular necrosis, infection, heterotopic ossification,and loss of range of motion.Conclusions: In capitellum fractures, anatomical reductionshould be ensured so as to provide elbow biomechanics.
