İyatrojenik pnömotoraks: Eğitim problem mi üretiyor?

Özet

Amaç: Tanı veya tedavi amaçlı invazif girişimlerin artmasına paralel olarak iyatrojenik pnömotorakslı olgularla sıklıkla karşılaşmaktayız. Bu çalışmada tanısal veya tedavi amaçlı girişimler sonucu pnömotoraks gelişen olgular incelenmiş ve tedavisi tartışılmıştır. Yöntem: 1 Ocak 2003-31 Aralık 2004 döneminde kliniğimizde pnömotoraks tanısı ile tedavi edilen 196 olgunun 53'ünde iyatrojenik'pnömotoraks saptandı. Olgular; yaş, cins, primer tanı, iyatrojenik travma nedeni, direnaj süresi ve tedavi sonuçları, ile analiz edildi. Bulgular: Olguların 26'sı kadın, 27'si erkek ve ortalama yaşları 44,16 bulundu (yaş aralığı:26 gün-82 yaş). Bu olgular, aynı dönemde tedavi edilen tüm pnömotorakslı olguların %27,04'ünü oluşturmaktadır. Uzayan hava kaçağı nedeniyle 6 olguda plörodezis uygulanmıştır Altı olguda (% 11,32) primer patolojilerine bağlı olarak pnömotoraks dışı nedenler le mortalite izlenmiştir. Diğer olguların 2-6 ay süren takıpleıınde ntıks saptanmamıştır. Sonuç: İyatrojenik pnömotaraks gelişimi eğitim hastanelerinde karşılaşılabilecek bir komplikasyondur. İnvazif girişimlerde pnömotoraks. nskı temel olarak uygulamacı doktorun tecrübesine dayanır. Tecrübe arttıkça komplikasyon riski azalmaktadır. Pnömotoraks gelişme riski olan uygulamalarda eğitim süreci daha uzun tutulmalı ve araştırma görevlilerinin eğitimi deneyimli uzmanlar refakatinde yapılmalıdır

Abstract

Purpose: Etiological factors that cause iatrogenic pneumothorax were evaluated and the risk factors for iatrogenic pneumothorax in various diagnostic procedures and clinical applications were emphasized. Methods: From January 2003 to December 2004, we treated 196 patients with pneumothorax. Fifty-three of them were found to have iatrogenic pneumothorax. Patients age, gender, primary diagnoses, etiology of iatrogenic pneumothorax, time of drainage and treatment results were analyzed. Results: Mean age of the 53 patients (26 women, 27 men) was 44.16 (range: 26 days-82 years). That was 27.04% of all pneumothorax patients. Mean time drainage was 8 days. Pleurodesis was applied to 6 patients with persistent air leak. Six patients (11.32%) died because of primary pathologies. No recurrence was seen in the other patients during the 2 to 6 months of the survey. Conclusion: With the increased number of thoracic procedures, the incidence of iatrogenic pneumothorax could be high in teaching hospitals. This depends on the experience of the doctor; therefore, we think that the procedures that may cause pneumothorax should be taught by experienced staff over longer periods.
