Polikliniğimize vajinit yakınmasıyla başvuran hastalarda vajinal akıntı etkenlerinin araştırılması

Özet

Amaç: Enfeksiyöz vajinitlerin en önemli üç etkeni bakterice, vajinozis, trikomonyazis ve vulvovajinal kandîdiyazistir. En sık izlenen semptomların ise; vajinal akıntı, kaşıntı ve yahfna hissidir. Gereç ve Yöntem: Çalışmamıza, polikliniğimize vajinal akıntı yakınma-sıyla başvuran 100 hasta alındı. Hastaların yaşı, son bir yıl içindeki partner sayısı, kullandığı doğum kontrol yöntemi, sigara, alkol kullanımı ve antibiyotik alımı kaydedildi. Vajinal akıntının özellikleri ve akıntıya eşlik eden kaşıntı, dizüri, alt abdominal ağrı, disparoni ve posfkoital kanama yakınmaları olup olmadığı sorgulandı. Vajinal akıntı örnekleri, posterior forniksden iki steril eküvyon ile transport besiyerine alındı. Birinci eküyyon ile EMB, kanlı ağara ve Sabouraud besi yerine ekim yapıldı.İkinci eküvyondaki örnek direk mikroskopi ile incelendi Bulgular: Vajinal akıntı etkeni olarak 42 hastada kandida dokuz hastada G. vaginalis, sekiz hastada E. coli, iki hastada T vaginalis iki hastada S aureus ve bir hastada Klebsiella saptandı. Hastalarımızdan 36'sıni vajinal örneklerinde herhangi bir etken saptanamadı ve normal vajen florası-bakterileri üredi. Vajinal akıntı etkenleri ile hastaların yaş grupları uygulanan korunma yöntemleri, geçirilmiş vajina öyküsu varlıgı spesifik, yakınmaları ve fizik muayene bulguları arasında istatistiksel olarak anlamlı fark bulunmadı (p>0,05). Sonuç: Vajinit etkenlerinin gerçek prevelanslannın oıtaya çıkfûası için daha geniş kapsamlı araştırmaların yapılması ve uygun değerlendirme yöntemlerinin kullanılması gerekmektedir

Abstract

Purpose: The three most important factors causing infectious vaginitis are bacterial vaginosis, trichomoniasis and vulvovaginal candidiasis. The most frequent symptoms are vaginal discharge, itching and burning sen¬sation. Methods: The patient sample of our study consisted of 100 patients who had consulted us with the complaint of vaginal discharge. The age of patients, the number of partners during the last year, the method of contraception, smoking and alcohol consumption and the antibiotics intake of the patients were recorded. Along with the properties of vaginal discharge, the presence of itching, dysuria, abdominal pain, dyspareunia and postcoital bleeding was investigated. Vaginal discharge samples taken from the posterior fornix by two sterilized swabs were put into the transport medium. The first swab was sent, in EMB, blood agar and Sabouraud medium. The sample from the second swab was examined by direct microscopy. Results: Candida was found to be the infecting agent in 42 patients, G. vaginalis in 9, E. coli in 8, T. vaginalis in 2, S. aureus in 2 and Kleibsiella in 1, all having vaginal discharge. In 36 of the patients, no agents were grown from vaginal discharge samples and normal vaginal flora resulted from cultures. There were no statistically significant differences between vaginal, discharge causes and the patients age, the method of contraception, previous vaginitis incidents, specific complaints or the findings obtained as a result of the physical examination (P>0.05). Conclusion: It is necessary to conduct more comprehensive studies and to use suitable evaluation methods to determine the actual prevalence of the factors causing vaginitis.
