Özet
Somut ve objektif veriler ile ölüm nedeni belirlenemeyen tüm olgular,adli ölüm olgusu olarak değerlendirilebilir. Bu olgularda ölüm sebebinin aydılatılması için tüm olanakların kullanılması şarttır. Otopsi işlemi ise, ölüm sebebi ve mekanizmasını ortaya koymada en önemli yöntemdir. Adli ölüm dolaylarında hangi nitelikteki olgulara adli otopsi yapılması gerektiği soruşturmayı yürüten savcı ve muayeneyi yapan bilirkişi hekimin insiyatifındedir. Bu 3 nedenle savcı ve hekimin deneyim ve bilgi düzeyleri değiştikçe benzer olgular-ffda farklı kararlar verildiği gözlenmektedir. Amaç: Bu çalışmanın amacı, 1992-2004 yılları arasında Gazi Hastanesi'nde adli ölü muayeneleri yapılan ölüm olgularının nedenleri ve ölüm nedenlerine ŞgÖre verilen otopsi kararlarındaki farklılıkları araştırmaktır.Gereç ve Yöntem: 1992-2004 yılları arasında, Gazi Hastanesi'ne ölü olarak T getirilmiş 338 adli olgu ile hastaneye getirildikten sonra ölen 464 adli olgunun Cumhuriyet Savcıları ve Adli Tıp Uzmanı hekimler tarafından düzenlenmiş Sadli ölü muayene tutanaklarına, hastane arşivinden ulaşılmış ve olgular, yaş, cinsiyet, ölüm nedeni ve otopsi yapılıp yapılmadığı yönünden retrospektif olarak incelenerek analiz edilmiştir. Bulgular: Olguların 539'u (%67.2) erkek, 263'ü (%32.8) kadındır. Yaş ortalamaları 38.19 ± 11.82'dir. Adli ölümlerin %83.9'una (673) otopsi yapılmamış, %16'ine (129) otopsi yapılmıştır. Adli ölümlerde ilk sırayı %64.5 (517) ile trafik kazaları almıştır. Bunu %7.5 (M) ile yüksekten düşmeler, %4.7 (38) ile şüpheli ölümler izlemektedir. Sonuç: Çalışmamız, cinayet orijinli ya da şüpheli ölüm olaylarında Cumhuriyet Savcıları ve Adli Tıp Uzmanı hekimlerin otopsi kararını daha kolay verdiklerini, ancak başlangıçta kaza ve intihar sonucu ölüm görüntüsü veren olgularda jiötopsi yapmaktan çok ölü muayenesi ile yetindiklerini düşündürmektedir.
Abstract
Introduction: All deaths without any concrete and objective proof of cause can be considered forensic death cases. In these cases, it is essential to use all facilities to clarify the cause of death. Autopsy is the most important method for determining the cause of death and the mechanism. The decision of whether an v- autopsy is necessary is made by the prosecutor conducting the investigation and the expert physician who performed the death investigation. As a consequence, in similar cases, with increasing knowledge and experience, the prosecutor and .physician can arrive at different decisions.Purpose: The aim of this study was to evaluate the differences between causes of death and autopsy decisions with regard to the actual causes of death among j. tnedico-legal death investigations performed in Gazi University Medical Fa-V culty Gazi Hospital. Methods: The records of 802 forensic death reports submitted by Republic Prosecutors and forensic medicine expert physicians were evaluated retrospe-| çtively. Of these cases, 338 were dead on arrival and 464 of them died after i. arrival at Gazi Hospital between 1992 and 2004. Records were obtained from i Gazi Hospital archives. Data on age, gender, cause of death and whether an autopsy was performed were evaluated and analyzed. Results: 539 of the subjects (67.2%) were male and 263 (32.8%) were female, sfhe mean age was 38.19 ± 11.82. An autopsy was performed in 16.11% of jyfbrensic cases (129). Road accidents ranked first, with 64.5% (517), among the forensic death cases. Falling from heights and suspicious deaths followed (7.5 |v|% and 4.7%, respectively). ftyonclusion: It appears that Republic Prosecutors and forensic medicine ex-Kiperts decide on an autopsy more easily in cases of murder and suspicious deifs Wnereas in cases that seem to be accidents or suicides, they are satisfied with the death examination rather than with an autopsy.
