Test-retest reliability of hand-held dynamometer and manual muscle strength measurements in amyotrophic lateral sclerosis

Amiyotrofik lateral sklerozda el-tutuşlu dinamometre ve manuel kas kuvvet ölçümlerinin test-tekrar test güvenilirliği Amaç: Bu çalışma, ambuiasyonunu sürdürebilen amiyotrofik lateral skleroz (ALS)'lu hastalarda manuel kas testi (MKT) ve el-tutuşlu dinamometre (ETD) ölçümlerinin test-tekrar test güvenirliğini göstermek amacıyla planlandı. Yöntem ve gereç: Kırk bir ALS hastasında, 13 kas grubu bilateral olarak MKT ve ETD yöntemiyle ölçüldü ve ölçümler 7-10 gün sonra tekrar edildi. Sonuçlar: Test-tekrar test analizinde, ETD mükemmel test-tekrar test güvenirlik gösterdi (ICC puanları: 0.87-0.99). MKT, omuz ekstansörleri (0.57), dirsek ekstansörleri (0.39) ve el bileği fleksörleri (0.53) dışında "iyi" ile "yüksek" arasında değişen test-tekrar test korelasyon katsayıları gösterdi. MKİ ölçümünün test-tekrar test güvenirlik katsayıları 0.39 ile 0.97 arasındaydı. Her iki yöntemin de test ve tekrar test kuvvet değerleri anlamlı olarak farklı değildi (p>0.05). Tartışma: Çalışmamızın sonuçları MKT ve ETD'nin aynı deneyimli ölçümcü tarafından uygulandığında güvenilir olduğunu gösterdi. Bununla birlikte, ETD'nin ALS'lu hastaların kas kuvvetinin değerlendirilmesinde daha güvenilir bir ö.lçüm yöntemi olduğu ve özellikle hastalığın başlangıç aşamalarındaki klinik çalışmalarda kullanılabileceği sonucuna varıldı.
Abstract
Purpose: This study was planned to determine the test-retest reliability of manual muscle testing (MMT) and hand-held dynamometer (HHD) measurements in community-ambulatory patients with amyotrophic lateral sclerosis (ALS). Material and methods: Thirteen muscle groups were tested bilaterally using both MMT and HHD methods on 41 ALS patients and then repeated the tests 7-10 days later. Results: Test-retest analysis for HHD showed excellent test-retest reliability (ICC scores ranging from 0.87 to 0.99). MMT demonstrated test-retest correlation coefficients ranged from "good" to "high" except for shoulder extensors (0.57), elbow extensors (0.39) and wrist flexors (0.53). The test-retest reliability coefficients of MMT testing were between 0.39-0.97. The strength values between test and retest were not significantly different for both MMT and HHD (p>0.05). Conclusion: Our results showed that both methods of strength testing to be reliable when the applied the same experienced rater. Also, it concluded that, HHD is more reliable measurement method in the assessment of muscle strength in patients with ALS and can be used in clinical trials especially at the initial stages of disease.
