Postoperatif erken dönemde derin solunum ve insentif spirometrenin etkinliğinin araştırılması

Özet

Amaç: Derin solunum ve insentif spirometre'nin (IS) solunum kas fonksiyonu, kan gazları ve hemodinamik parametreler üzerindeki etkilerinin belirlenmesidir. Gereç ve yöntem: Abdominal cerrahi uygulanan ve yoğun bakım ünitesinde tedavi edilen 60 olgu üzerinde gerçekleştirildi. Olgular rasgele örnekleme yöntemiyle iki gruba ayrıldı. Grup 1 olgularına; derin inspirasyon, zorlu ekspirasyon tekniği ve öksürme tekniklerinden oluşan fizyoterapi programı, Grüp> 2 olgularına; IS ile inspirasyon, zorlu ekspirasyon tekniği ve öksürme tekniklerinde^, pluşan fizyoterapi programı, saatte bir 5-10 tekrarla 6 saat süresince uygulandı. Kalp atım hızı (KAH), ortalama arteriyal basınç (OAB), SpOz her tedavi seansı öncesi ve' sbrfrbsı ölçüldü. Tedavi öncesi ve sonrası FEV,, FVC, FEV, / FVC ve ağız içi maksimum İnspirasyon (Plmax) ve ekspirasyon (PEmax) basıncı ölçüldü. Kan gazı değerleri tedavi öncesi, tedavi sonrası 3. ve 6. saatte kaydedildi. Sonuçlar: Her iki grupta tedavi sonrası 6. saatte Pa02 değerindeki artışın tedavi öncesine göre istatistiksel olarak anlamlı olduğu bulundu (p0.001). Tartışma: Abdominal cerrahi sonrası derin solunum ve IS'nin hemodinamiyi olumsuz etkilemediği, oksijenasyonu artırdığı, bu nedenle postoperatif göğüs fizyoterapisinde birbirinin alternatifi olabilecek yöntemler olduğu sonucuna varılmıştır.

Abstract

Purpose: This study has been planned to compare the effects of deep breathing and incentive spirometry (IS) on respiratory muscle function, arterial blood gases and hemodynamical parameters. Material and methods: This study was carried out with 60 patients who had abdominal surgery and were treated in the intensive care unit. Patients were divided randomly into two groups. Group 1 was applied physiotherapy program deep inspiration, forced expiration and coughing techniques, Group 2 was applied physiotherapy program included IS, forced expiration and coughing techniques. Both programs were performed every hour with 5-10 repetitions during 6 hours. Heart rate, mean arterial pressure and Sp02 calculated before and after each treatment period. FEV,, FVC, FEV, / FVC, Plmax and PEmax were measured before and after treatment. Arterial blood gases were recorded before and at 3rd and 6th hours after treatment. Results: We found that PaOz values increased significantly at both groups after 6th hours treatment (p0.001). Conclusion: We found that after abdominal surgery deep breathing and IS do not negatively effect hemodynamic parameters and it increases oxygenetation. It is concluded that there are two different methods, which can be each others' alternative for postoperative chest physiotherapy.
