Majör depresif hastalarda kısa ve uzun dönem ilaç tedavisinin etkinliği ile nesfatin-1, grelin ve nitrik oksit seviyesi arasındaki ilişkinin belirlenmesi

Özet
Amaç: Majör Depresif Bozukluk (MDB) ya ş am kalitesini azaltan önemli bir mental hastalıktır. Buhastalı ğın geli ş mi ş ve geli ş mekte olan ülkelerde görülme sıklı ğ ı gittikçe artmaktadır. Buçalı ş mada MDB ve hormonal de ğ i ş imler arasında olabilecek ili ş kiyi anlayabilmek için, nesfatin-1,nitrik oksit ve grelin düzeylerini 9 aylık tedavi öncesi ve sonrası ara ş tırmayı amaçladık.Gereç ve Yöntem: Bu çalı ş maya toplamda 60 ki ş i (30 MDB hasta 39.8±2 yıl, 64.9±1.8 kg; 30sa ğ lıklı kontrol 36.6±3 yıl, 70.6±2.3 kg) katılmı ş tır. Açlık venöz kan örnekleri bazal, 3. ay ve 9.ayda olmak üzere 3 defa alınmı ş tır. Serum nesfatin-1, total grelin ve nitrik oksit düzeyleri ELISAyöntemi ile ölçülmü ş tür.Bulgular: MDB hastalarında bazal nesfatin-1 seviyesi (73.4±1.3 ng/mL) kontrol grubundan(13.5±3.2 ng/mL) anlamlı olarak yüksek bulundu (P=0.001). Bununla birlikte çalı ş manın 3. ayında37.6±4.3 ng/mL (P=0.001) ve 9. ayında 22.3±3.5 ng/mL nesfatin-1 seviyesi anlamlı olarak azaldı(P=0.001). MDB hastalarının bazal nitrik oksit (NO) seviyesi (2.7±0.6 nmol/L) kontrol grubundananlamlı olarak dü ş ük bulundu (6.5±0.4 nmol/L) (P=0.001). NO seviyesi ilk 3. ayda 3.5±0.7 nmol/Lye (P=0.04) ve 9. ayda ise 4.1±0.9 nmol/L ye artı ş gösterdi (P=0.04). MDB hastalarının bazalgrelin seviyesi (44.5±3.7 pg/mL) kontrole göre anlamlı dü ş ük bulundu (310.3±35.6 pg/mL)(P=0.001), fakat 3. ay ve 9. aylarda grelin seviyesinde anlamlı de ğ i ş iklik gözlenmedi.Sonuç: Bazalde, yüksek nesfatin-1 ve dü ş ük NO seviyeleri ve tedavi ile nesfatin-1 deki anlamlıazalma ve NO deki anlamlı yükselme tedavi sonrası hastaların durumlarının de ğ erlendirilmesindeyardımcı olabilecektir. MDB ve nesfatin-1 ve NO arasındaki ili ş kinin anla ş ılabilmesi için ileriçalı ş malara ihtiyaç duyulmaktadır.
Abstract
Objective: The major depressive disorder (MDD) is a serious mental disease reducing thequality of life. Its prevalence is increasing in developed and developing countries. In the presentstudy, to understand the relationships between MDD and hormonal changes, we were purposedto examine the nesfatin-1, nitric oxide and ghrelin levels before and after the 9 months oftreatment. Materials and Methods:Total of 60 subjects (30 MDD 39.8±2 yr, 64.9±1.8 kg; 30 healthycontrol 36.6±3 yr 70.6±2.3 kg). Fasting venous blood samples were taken three times: At thebasal, at the end of the three and nine months of the study. ELISA method was used to analyseserum nesfatin-1, nitric oxide and ghrelin levels.Results The basal nesfatin-1 level in MDD was found to be significantly higher (73.4±1.3 ng/mL)than the control levels (13.5±3.2 ng/mL) (P=0.001). However, it decreased significantly to37.6±4.3 ng/mL after 3 months (P=0.001) and 22.3 ±3.5 ng/mL after 9 months (P=0.001). Thebasal NO level in MDD (2.7±0.6 nmol/L) was found to be significantly lower than the control(6.5±0.4 nmol/L) (P=0.001). It increased to 3.5±0.7 nmol/L in the first third month (P=0.04) andcontinued to increase after 9 months 4.1±0.9 nmol/L (P=0.04). In the MDD, basal ghrelin level(44.5±3.7 pg/mL) was found to be significantly lower than the control (310.3±35.6 pg/mL)(P=0.001) but it did not change significantly after 3 (42.5±3.7 pg/mL) and 9 months (40.8±2.6pg/mL) of the study.Conclusion: Observation of the markedly high nesfatin-1 and markedly low NO levels at thebasal and significant decrease in nesfatin-1 and increase in NO levels to their control level aftertreatment may be help to understand the patients current status. Further studies are needed tounderstand this relationship between MDD and nesfatin-1 and NO levels.
