Apendektomi Yapılan Çocuklarda İntraoperatif Kültür ve Antibiyogram Sonuçlarının Değerlendirilmesi

Özet
Amaç: İntraabdominal enfeksiyonların önemli nedenlerinden biri perfore apandisittir. Perfore apandisitte elde edilen mikrobiyoloji bulguları, toplum kökenli intraabdominal enfeksiyonlarda sık görülen mikroorganizmaları yansıtmaktadır. Bu çalışmada, apendektomi yapılan çocuklarda intraoperatif kültür ve antibiyogram sonuçları irdelenerek toplum kaynaklı intraabdominal enfeksiyonlarda bölgesel direnç paternimizin belirlenmesi amaçlandı. Gereç ve Yöntem: Çalışmaya Ocak 2013-2014 tarihleri arasında Çocuk Cerrahi kliniğinde apandisit tanısıyla apendektomi yapılan, yaşları 2-16 yıl arasında 44 hasta alındı. Hastalardan intraoperatif olarak alınan 1-10 mL peritoneal mayi veya apendiküler doku kanlı agar ve EMB agar plaklarına ekildi. İzole edilen bakterilerin antibiyotik duyarlılığı disk difüzyon yöntemi ile CLSI önerileri doğrultusunda araştırıldı. Sonuçlar duyarlı ve dirençli olarak değerlendirildi. Bulgular: Çalışmamız süresince kliniğimizde 44 hastaya apendektomi uygulandı. Ameliyat edilen 20 hastada (%45) perfore apandisit, 8 hastada (% 18) akut apandisit, 16 hastada (%37) ise süpüratif apandisit saptandı. Hastalardan alınan intraoperatif kültürde 33 (%75) hastada üreme tespit edildi. On bir (%25) hastada ise üreme saptanmadı. Etken patojen olarak; 27 (%82) Escherichia coli, 2 (%6) Streptococcus spp. izole edildi. Enterobacter cloacae, Klebsiella spp., Enterococcus spp. ve Micrococcus spp. birer (%3) olguda üredi. E. coli şuşlarının 3'ünde (%11) ESBL pozitifliği saptandı. İzole edilen Gram negatif bakterilerde ampisilin-sulbaktam direnci %61 (n= 17), kinolon direnci %20.7 (n= 6) bulundu. Gram pozitif mikroorganizmalarda penisilin, metisilin ve vankomisin direnci saptanmadı. Sonuç: Bu çalışma bölgemizdeki toplum kaynaklı intraabdominal enfeksiyonlarda etken ve direnç profilimizi belirlemiş olması nedeniyle ayrı bir önem kazanmıştır. Ayrıca intraabdominal enfeksiyonlarda ampirik tedavi yaklaşımına ışık tutacaktır. Bu konuda ülkemizde yapılmış yeterli çalışma bulunmamaktadır
Abstract
Objective: One of the major causes of intra-abdominal infections is perforated appendicitis. The microbioloy findings obtainedby perforated appendicitis reflect microorganisms common in community- acquired intra-abdominal infections. In this study, it is aimed to determine regional resistance pattern in community- acquired intra-abdominal infections by examining intraoperative culture and antibiogram results gby children who underwent appendectomy. Materials and Methods: 44 patients between the ages of 2-16 years underwent appendectomy with the diagnosis of appendicitis in clinical Pediatric Surgery between January 2013- 2014 were enrolled to the study. 1-10 mL peritoneal fluid or appendicular tissue taken from patients intraoperatively were cultured to blood agar and EMB agar plates. Antibiotic susceptibility of isolated bacteria was investigated with disk diffusion method according to CLSI suggestions direction. Results were considered as sensitive and resistant. Results: Appendectomy was performed by 44 patients in our clinic during our study. Perforated appendicitis was found in 20 patients (45 %), acute appendicitis in 8 patients (18%), suppurative appendicitis in 16 patients (37 %) were identified. Reproduction were found on 33 (75%) in intraoperative cultures from patients. Reproduction were not found in 11 (25 %) patients. By 27 (82%) Escherichia coli, 2 (6%), Streptococcus spp. was isolated as effective pathogen. Enterobacter cloacae, Klebsiella spp., Enterococcus spp. and Micrococcus spp growth was observed on single case for each (3%). ESBL positivity was detected in 3 (11%) of E. coli strains. Ampisilin-sulbactam resistance by 61% (n= 17), quinolone resistance by 20.7% (n= 6) was found in isolated Gram-negative bacteria. Conclusions: This study has acquired particular importance because it determines factor and resistance profile in community- acquired intra-abdominal infections. It also shed light on the empirical treatment of intra-abdominal infections. We cannot find enough work in our country about this subject and we need extensive research with a larger number of patients
