Özet

Diyafragma hernisi sıklıkla Bochdalek ve Morgagni olarak iki tipte gorulur. Anterolateral yerlesimli bu nadir olgu gec tanı konulması ve ciddi beslenme sorunlarına yol acması nedeni ile sunulmuştur. Beslenme sonrasında siskinlik ve aralıklı kusma sikayetleri olan 15 yasında erkek hasta hematemez nedeniyle tarafımıza konsulte edildi. Hastanın genel durumu duskun, dehidrate, batını cokuktu ve sol hemitoraks bazalde solunum sesi alınamıyordu. PA Akciger grafisinde sol akciger bazalde hava sıvı seviyesi mevcuttu. Cekilen toraks BT'de midenin fundus kısmının diyafragma defektinden toraksa gecmis oldugu belirlendi. Laparotomide sol diyafragmanın anterior-lateral kısmında midenin fundus ve korpusunun bir kısmı ile omentumun sol hemitoraksa herniye oldugu defekt primer onarıldı. Per-operatif ve post-operatif herhangi bir komplikasyona rastlanmadı. Hasta cerrahi sonrası kontrollerinde sorunsuz izlenmektedir. Tipik diyafragma hernilerinin buyuk cogunlugu erken cocukluk doneminde tanı alır. Atipik yerlesimli defekti olan bu olgu ise oldukca gec tanı almıstır. Gastrik sikayetler ve beslenme geriligi olan hastalarda ayırıcı tanıda diyafragma hernisi akılda tutulmalıdır.

Abstract

Diaphragmatic hernia occurs frequently in two types; Bochdalek and Morgagni. This anterolateral localized rare case, causing serious nutritional problems was presented due to its late diagnosis. Fifteen-year-old boy with history of intermittent vomiting and failure to thrive was consulted to our pediatric surgery unit for haematemesis and dehydration. On general examination, there was no breath sounds on left bottom side of the thorax and scaphoid abdomen was seen. The Chest X- ray revealed air-fluid levels at left bottom hemi-thorax. Thorax-BT defined the pathology as diaphragmatic hernia containing the fundus of the stomach. Laparatomy revealed defect on left anterio-lateral part of diaphragma holding portion of the gastric fundus corpus and omentum by fibrotic bands, which was repaired primarily. Oral feeding began 48 hours post operatively and patient was discharged on seventh day without any complications. Although the majority diaphragmatic hernias give signs in early childhood, this case has been identified late as it was an atypically localized defect. Diaphragmatic hernia should be kept in mind in patients with longstanding gastric complaints and malnutrition.
