Özet: Erişkin hastalarda kronik subdural hematom (SDH)'un tedavisi farklı cerrahi tekniklerle yapılmaktadır. Son yıllarda kraniotomiden ziyade burr hole veya twist-drill ile tedavi yönünde eğilim vardır. Burr hole ve twist-drill'in morbidite ve mortalitesi düşük olup sonuçları eşittir. Bununla birlikte birçok nöroşirürjiyen bu patolojinin tedavisi için kraniotomiyi, burr hole tercih etmektedir. Kronik SDH'un tedavisinde burr hole kraniostomi ve kapalı sistem drenajın etkisini belirlemek için 104 hastanın kayıtlan incelendi. Seksenbeş (%82) hasta mükemmel iyileşme gösterirken, 9 hasta önemli düzelme göstermedi. Tüm nedenlerden total mortalite %9.6, rekürrens oranı %20.2, pnömosefalus oranı %18.2 olup, 8 (%7.6) hasta ilave cerrahi işlemi gerektirdi. Kraniotomi ve twist drill'e karşı burr hole'nin üstünlüğünü savunan yayınlar bu çalışma ile doğrulanmış olup, kronik SDH'lu hastaların tedavisi için burr hole güvenilir ve emin bir tekniktir.
Abstract
Summary: The management of chronic subdural hematoma in the adult patient is achieved with a variety of different surgical techniques. The trend in recent years has been toward treatment with burr holes or twist-drill holes rather than craniotomy. Burr holes and twist-drill holes offer equivalent efficacy and lower morbidity and mortality. However, many neurosurgeons feel that craniotomy is superior to a burr hole for the management of this condition. The case records of 104 patients were reviewed to ascertain the efficacy of craniostomy and continuous closed-system drainage for the treatment of chronic subdural hematomas. Eighty-five (82%) patients achieved an excellent outcome, and 9 (8.6%) had no significant improvement. The total mortality from all causes was 9.6%. There was a 20% recurrence rate and a 18% pneumocephalus rate. Eight patients (7.6%) required additional operative procedures. Previous reports concerning the superiority of burr holes over craniotomy are substantiated by this review. Burr hole remains a valuable and safe technique for the management of patients with chronic subdural hematoma.
