Özet

1987-1997 yılları arasında Erciyes Üniversitesi Tıp Fakültesi Üroloji Anabilim Dalına idrar -kaçırma şikayeti ile başvuran ve stres enkontinans tanısı konan 26 hastanın 14 'üne Gittes, 12 'sine Marshall-Marshetti-Krantz operasyonu uygulandı. Gittes grubunda izlem süresi 32(3-69) ay ve hastanede kalış süresi dört (3-10) gün idi. Bu olguların izlemlerinde birinci yıl %95, ikinci yıl %86 oranında başarı sağlandı. Operasyon sonrası yalnızca bir olguda makroskobik hematüri ve dört olguda geçici işeme güçlüğü gömldü.O Marshall-Marshetti-Krantz grubunda ise takip süresi 42(17-95) ay ve hastanede kalış süresi sekiz (7-12)gün idi. Bu grupta birinci yıl %98, ikinci yıl %90 başarı oranı sağlandı. Bir olguda retropubik hematom ve 'üç olguda operasyon sonrası geçici işeme güçlüğü ile karşılaşıldı. Çalışmamızda Gittes tekniği ile onarımın kolay ve çabuk oluşu, hastanede kalış süresinin kısalığı ve düşük morbidite gibi özellikle re sahip olduğu, dolayısıyla öncelikli olarak tercih edilebileceği sonucuna varıldı.

Abstract

: Twenty six cases of stress incontinence were diagnosed at the Urology Clinic of Erciyes University Faculty of Medicine between 1987-1997 and operated. Gittes technique was used for 14 of them and Marshall-Marshetti-Krantz for 12. In Gittes group length of the follow-up of the follow-up was 32(3-69) months and the hospitalisation time was 4(3-10) days; the success rates were 95% after one year and 86% after two years. Postoperative gross hematuria was observed only in one case. In Marshall- Marshetti Krantz group length of the follow up was 42(17-95) months and hospitalization time was eight(7-12) days; the success rates were 98% after one year and 90% after two years. Retropubic hematoma was observed in one case and temporary voiding difficulty in three cases postoperatively. In conclusion, due to easy manipulation, short hospitalization time and low morbidity, the Gittes bladder neck suspansion can be preferred to Marshall Marshetti Krantz operation for stress incontinence.
