Kayseri Askeri Hastanesi Göz Kliniği 'nde ve serbest pratiğimizde Mayıs 1994-Nisan 1996 tarihleri arasında 142 hastanın 165 gözüne planlı ekstrakapsüler katarakt ekstraksiyonu (PEKKE)+arka kamara göz içi lensi (AK-GİL) implantasyonu ameliyatı uygulandı. Altmış yedi hastanın 81 gözüne, preoperatif olarak aksiyel uzunluk, keratometri ölçümleri yapıldı ve emetropiye göre GÎL gücü hesaplandı. Yetmişbeş hastanın 84 gözüne standart güçte (+21.5-/.+0.5 diyoptri) GÎL kondu. Sferik eşdeğerler kriter olarak alındı. Biometrik ölçüm yapılan olgularda %93.2, ölçüm yapılmayanlarda %77.4 oranında, kabul edilebilir psödofakik refraksiyon aralığı olarak kabul edilen +1 ile -2 dioptri aralığında, refraksiyona ulaşılmıştır. Orta dereceli miyop ve hipermetroplarda preoperatif GÎL ölçümünün postoperatif refraksiyonu fazla etkilemediği görülse bile yüksek risk gruplarında bu işlem önem kazanmaktadır.

Abstract

One hundered and sixty-five eyes of 142 patients underwent planned extracapsular cataract extraction (PECCE) with implantation of posterior chamber intraocular lens (IOL). In the first group, (81 eyes of 67 patients) the axial length of the eye was measured ultrasonographically, keratometry was performed and IOL power required to produce emmetropia was calculated preoperatively. The second group (84 eyes of 75 patients) received a standard power IOL of +21.5 dioptres (-1+0.5 dioptres depending upon availability). The postoperative refraction (spherical equivalent) was calculated for each patient. The percentages of postoperative refraction values between +1.0 and -2.0 dioptres were 93.2% for the eyes in the first and 77.4% in the second goup. Although preoperative calculation of IOL power does not seem to affect the postoperative refraction considerably in average myopia and hypermetropia, this procedure gains importance in high risk groups.
