Hemoptizili olgularda yüksek rezolüsyonlu bilgisayarlı tomografi ve bronkoskopinin tanısal değeri

Hemoptizili olgularda yüksek rezolüsyonlu bilgisayarlı tomografi (YRBT) ve fiberoptik bronkoskopi (FOB)'nin tanısal değerini saptamak için ileriye dönük bir çalışma yapıldı. Hemoptizili 50 olgu (32 erkek, 18 kadın) YRBT ve FOB ile incelendi. Etiyolojik olarak kronik obstruktif akciğer hastalığı (10 olgu %20), akciğer tümörü (altı olgu %12), pnömoni (yedi olgu %14), tüberküloz (altı olgu %12), bronşektazi (beş olgu %10), kriptojenik (12 olgu %24) ve çeşitli nedenler (dört olgu %8) bulundu. Bronkoskopide üç tümör olgusu saptanırken YRBT ile altı akciğer tümörü gösterildi. Üç tümör FOB alanının dışında idi. YRBT santral akciğer tümörlerinin intra ve ekstraluminal yayılımını saptamada, periferik tümörleri, kaviter tüberküloz, bronşektazi ve pnömoni gibi inflamatuvar hastalıkları göstermede duyarlı iken, bronkoskopi erken mukozal anormallikleri, bronşit ve skuamoz metaplazileri göstermede duyarlı bulundu. Sonuç olarak hemoptizili olgularda YRBT noninvaziv bir yöntem olarak bronkoskopiden önce yapılmalı ve gerekirse olgular bronkoskopiye gönderilmelidir.
Abstract
A prospective study was performed to detect the diagnostic value of high resolution computed tomography (HRCT) and fiberoptic bronchoscopy (FOB) in patients with hemoptysis. Fifty patients (32 men, 18 women) with hemoptysis underwent HRCT and FOB. Etiologies included chronic obstructive lung disease (10patients, 20%), lung tumor (six patients, 12%), pneumonia (seven patients, 14 %), tuberculosis (six patients, 12%), bronchiectasis (five patients, 10 %), cryptogenic (12 patients, 24 %) and miscellaneous causes (four patients, 8 %). Six lung tumors were depicted by HRCT while three of tumors were detected by FOB. Three of the tumors were beyond bronchoscopic range. It is concluded that HRCT is sensitive in identifying both the intra and extraluminal extent of central lung tumors, in demonstrating peripheral tumors, inflammatory diseases such as cavitary tuberculosis, bronchiectasis, and pneumonia, while bronchoscopy is sensitive to early mucosal abnormalities, bronchitis, and squamous metaplasia. Finally, HRCT must be obtained prior to FOB as a noninvasive method in patients with hemoptysis;. if necessary, the patients should be referred to bronchoscopy.
