Ottowa ayak bileği kriterlerinin acil servislerde uygulanması: Gereksiz radyografi istekleri azaltılabilir

Özet
Amaç: Bu çalışmada Ottawa ayak bileği kriterlerinin (OABK) radyolojik tetkik oranını azaltıp azaltmadığının ortaya konulması amaçlandı. Gereç ve yöntem: Çalışma, ayak bileği ve çevresinin yaralanması nedeniyle acil servise başvuran 198 erkek hasta üzerinde yapıldı. I. grupta (n, 103; ortalama yaş 21) OABK uygulanmadı. II. grupta ise (n, 103; ortalama yaş 21) OABK uygulanarak gereken vakalarda radyolojik tetkik istenip istenmemesine karar verildi. Bulgular: Kriterlerin uygulanmadığı I. grupta grafi istenen hasta sayısı 79 (%76,6) ve kırık çıkan hasta sayısı dört, grafi istenen hastalarda kırık bulunma oranı %5,1 olarak bulundu., kriterlerin uygulandığı II. grupta ise grafi istenen hasta sayısı 20 (%21,0) ve kırık tespit edilen hasta sayısı yedi, grafi istenen hastalarda kırık bulunma oranı %35 olarak bulundu. Buna göre OABK'nin uygulandığı II. grupta grafi isteme oranında %72,5 azalma oldu. Sonuçlar, OABK'nin radyolojik tetkik isteme oranını azaltma durumu yönünden oldukça anlamlı idi (P < 0,001) ve sensitivitesi %100, spesifitesi %85 olarak bulundu. Sonuç: Bu çalışmanın sonucunda OABK uygulanmasıyla ayak bileği ve çevresi kırıklarının uygun bir şekilde değerlendirilebileceğini ve neredeyse hata payı olmadan radyolojik inceleme oranının azaltılabileceğini söyleyebiliriz.
Abstract
Purpose: The purpose of this study was to assess the clinical usefulness of the Ottawa Ankle Rules (OAR) when implemented in an emergency department. Material and methods: This study was performed in an emergency service on 198 consecutive male patients who suffered from trauma of the ankle region. In the first group (n, 103; mean age, 21), all patients were examined without using the OAR. In the second group (n, 95; mean age, 21), all patients were thoroughly examined using the OAR and the desicion as to whether a radiograph was necessary or not was given. Results: In the first group, there were 79 (%76.6) patients referred for radiography and only four patients were found to have fractures. The ratio of fractures in the patients radiographically evaluated was 5.1%. In the second group, there were 20 (21%) patients referred for radiography and seven patients were detected as having fractures. The ratio of fractures in the patients radiographically evaluated was 35%. The sensitivity for this examination was found to be 100% and its specifity, 85%. Conclusion: Implementation of the Ottawa Ankle Rules significantly reduced radiography requests (p < 0.0001) in our study and we believe that the OAR can adequately screen ankle fractures when implemented by physicians.
