Meme sitolojilerinin istatistiksel analizi ve histopatolojik tanılarla ilişkileri

Özet
Amaç: Meme lezyonlarının tanısında ince iğne aspirasyon sitolojisinin (İİAS) öneminin belirlenmesi amacıyla geriye dönük olarak 272 İİAS yeniden değerlendirildi. Gereç ve Yöntem: Sitolojik tanı 154 (% 56.6) hastada benign, 13 (% 4.8) hastada şüpheli, 32 hastada (%11.8) malign; 73 (%26.9) hastada ise yetersizdi. Bulgular: Hastaların 93'ü (%34.2) ve malign tanı alan hastaların tümü histopatolojik tanıları ile karşilaştırıldı.Yalancı negatif tanı alan iki hasta bulunmaktadır. Duyarlılık % 80.0, Özgüllük % 84.9 olarak bulunmuştur. Tümörün örneklenmesindeki hatalar, karsinom dışındaki diğer malignitelerde tecrübe eksikliği yalancı negatif tanının nedenidir. Sonuç: İİAS meme lezyonlarının tanısında ucuz ve güvenilir bir tanı yöntemidir.
Abstract
Aim: In order to investigate the reliability of the fine-needle aspiration cytology (FNAC) in the diagnosis of breast masses, 272 breast FNAC were evaluated retrospectively. Material and Methods: The cytologic diagnosis was benign in 154 cases ( 56.6 %), suspicious in 13 (4.8 %) , malignant in 32 (11.8 %) and insufficient in 73 (26.9 %) of the aspirates. Results: In 93 (34.2 %) of the cases and in 100 % of cytologically malignant smears, the aspirate was compared using histologic examination. False negative FNAC occurred in two cases. For the sample used the specificity was 84.9 %, and the sensitivity 80.0 %. Sampling error and inexperience in other malignancies than carcinoma were the causes of the false negative diagnosis. Conclusion: FNAC is an indispensable and reliable diagnostic tool in the management of breast lesions.
