Spontaneous fornix rupture due to obstructive ureteral stone

Özet

Üroloji pratiğinde renal forniksin spontan rüptürü ve idrar ekstravazasyonu ile çok nadir karşılaşılır. Bu yazıda, tıkayıcı üreter taşı nedeniyle aniden gelişen spontan forniks rüptürü ve idrar ekstravazasyonu olan 57 yaşındaki erkek hasta sunulmaktadır. Hastada herhangi bir travma olmaksızın ani başlangıçlı renal kolik ağrısı gelişti. Tam kan sayımı ve böbrek fonksiyon testleri normal sınırlar içindeydi. Tam idrar tetkikinde mikroskopik hematüri tespit edildi. Düz radyografide hiçbir üriner opasite yoktu. Üriner ultrasonografide, sol renal pelvis ve üreter genişlemişti ve sol üreterin distal ucunda yaklaşık 4 mm çaplı bir taş ile uyumlu hiperekoik görünüm mevcuttu. İntravenöz pyelografide sol renal pelvisin ve üreterin Grade 1 dilatasyonu ve peripelvik alanda kontrast madde ekstravazasyonu gözlendi. Spiral bilgisayarlı tomografi de sol pararenal alanda kontrast madde ekstravazasyonu gösterdi. Bu vakada, semptomları kontrol etmek ve ekstravazasyon ortadan kaldırmak amacıyla çift J stent kateterizasyonu yapıldı. Postoperatif ağrısı azaldı ve takipte alfa-bloker tedavisi başlatıldı. Ekstravazasyon geriledi ve takip ultrasonlarda hidronefroz kayboldu. İki hafta sonra, hasta taşın düştüğünü belirtti. Kateter çekildi ve hasta aynı gün taburcu edildi.

Abstract

Spontaneous rupture of the renal fornix and urinary extravasation are very rarely encountered in urological practice. In thepresent paper, a 57-year-old male patient who suddenly developed spontaneous rupture of the fornix and urinary extravasationdue to obstructive ureteral stone is presented. The patient developed a sudden onset of renal colic pain without any trauma. Hiscomplete blood count and kidney function tests were within the normal limits. Microscopic hematuria was detected on completeurinalysis. There was no urinary opacity on plain X-ray. On urinary ultrasonography, the left renal pelvis and ureter were dilatedand there was a hyperechoic appearance consistent with a stone approximately 4 mm in diameter at the distal end of the leftureter. Grade 1 dilatations of the left renal pelvis and ureter and extravasation of contrast material at the peripelvic area wereobserved on intravenous pyelography. Spiral computed tomography also showed extravasation of contrast material in the leftpararenal area. In the present case, double J stent catheterization was performed in order to control symptoms and eliminate ex -travasation. His postoperative pain decreased and alpha-blocker treatment was initiated at the follow-up. Extravasation regressedand hydronephrosis disappeared on follow-up ultrasounds. Two weeks later, the patient stated that he had passed the stone. Thecatheter was withdrawn and the patient was discharged on the same day.
