Kliniğimizde yatarak tedavi gören diabet hastalarımızın genel değerlendirilmesi

Özet
Bu çalışmada Eylül 98- Kasım 99 tarihleri arasında kliniğimizde yatarak tedavi gören diabetik hastalar retrospektif olarak incelendi. 304 hasta (211 kadın, 93 erkek, yaş ortalamaları 59 ± 15 yıl) yatarak tedavi görmüştür. Bu hastaların 58'i tip 1 diabetes mellitus, 246'sı tip 2 diabetes mellitus hastasıydı. Tip 1 diabetik hastaların % 46.55'i (27 hasta) diabetik ketoasidoz tanısı ile, sadece bir hasta ise hipoglisemi nedeniyle kliniği yatırılmıştır. Tip 2 diabetik hastaların % 8.10'u (20 hasta) hiperosmolar nonketotik koma, % 6.50'si (16 hasta) ise diabetik ketoasidoz nedeni ile kliniğe yatırılmıştır. Tip 1 diabetik hastaların, % 8.62'sinde retinopati, % 20.69'unda nöropati, % 29.31'inde mikroalbüminüri, % 13.79'unda makroalbüminüri, % 6.9'unda diabetik ayak, % 5.17'sinde hipertansiyon ve % 5.17'sinde koroner arter hastalığı mevcuttu. Tip 2 diabetik hastaların ise; % 70.32'sinde retinopati, % 61.38'inde nöropati, % 47.99'unda mikroalbüminüri, % 22.76'sında makroalbüminüri, % 10.57'sinde diabetik ayak, % 47.97'sinde hipertansiyon ve % 33.73'ünde koroner arter hastalığı mevcuttu. Hastaların % 31.57'sinde (96 kişi, n:304) hiperkolesterolemi, % 36.84'ünde (112 kişi, n:304) hipertrigliseridemi tespit edildi. Ortalama HbA1c ise % 10.14 ±3.41 olarak bulundu. Sonuç olarak diabet hastalarımızda akut ve kronik komplikasyonlar beklenenin üzerinde bulunmuştur. Hastaların hastalıkları ile ilgili bilgileri yeterli düzeyde değildir.
Abstract
In this study, diabetic patients treated in our in-patient clinic between September 98 and November 99 were studied retrospectively. 304 patients were treated in-patient clinic (211 females, 93 males, age average 59 ± 15 years) out of these patients 58 were type 1 Diabetes Mellitus (DM) and 246 were type 2 DM patients. 46.55 % (27 patients) of type 1 diabetic patients were admitted to in-patient clinic due to diabetic ketoacidosis and only one patient due to hypoglycaemia. 8.10 % (20 patients) out of type 2 diabetic patients were admitted to in-patient clinic due to hyperosmolar nonketotic coma, 6.50 % (16 patients) ones due to diabetic ketoacidosis. Out of type 1 diabetic patients there were retinopathy in 8.62 %, neuropathy in 20.69 %, microalbuminuria in 29.31 %, macroalbuminuria in 13.79 %, diabetic foot in 6.9 %, hypertension in 5.17 % and coronary artery disease in 5.17 %. Out of type 2 diabetic patients there were retinopathy in 70.32 %, neuropathy in 61.38 %, microalbuminuria in 47.99 %, macroalbuminuria in 22.76 %, diabetic foot in 10.57 %, hypertension in 47.97 % and coronary artery disease in 33.73 %. Out of patients there were hypercholesterolemia in 31.57 % (96 persons, n: 304), hypertrigliseridemia in 36.84 % (112 persons n:304). Mean HbA1c2 was found as 10.14±3.41 %. As a result, it has been found that acute and chronic complications in our diabetic patient were over expectations. Informations of the patient about their diseases were not sufficient.
