Özet
Herediter hemokromatozis otozomal resesif geçişli nadir bir metabolik hastalıktır. Temel bozukluk intestinal demir emiliminin artışı sonucu karaciğere, kalbe, eklemlere, pankreas ve diğer endokrin organlara demir birikimi sonucu gelişen organ disfonksiyonlarıdır. Tedavisinde klasik olarak flebotomiyle vücuttan demirin uzaklaştırılması önerilmektedir. Ancak tedavinin komplikasyonlar üzerine etkisi yeterince bilinmemektedir. 31 yaşında erkek hastamızda tanı sırasında karaciğer sirozu, diabetes mellitus ve hipogonadotrpik hipogonadizm saptandı. 6 ay süreyle 32 seans flebotomi uygulandı. 6 ay sonunda yapılan değerlendirmede hipogonadizm ve karaciğer fonksiyonlarında sınırlı iyileşme, buna karşılık diabette progresyon saptandı. Bu bulgular ileri evre hemokromatoziste pankreas hasarının geri dönüşümsüz olduğunu diğer sistemlere ait bozuklukların ise kısmen düzelebileceğini göstermiştir.
Abstract
Hereditary hemochromatosis is a rare autosomal recessive metabolic disease. The main disorders are organ dysfunctions that are developing as a result of excess intestinal iron absorption which leads to iron deposition in the liver, heart, joints and pancreas, and other endocrine organs. Classically the treatment contains the removal of iron from the body by phlebotomy. However the effects of treatment isn't well known. We established hepatic cirrhosis, diabetes mellitus and hypogonadotrpic hypogonadism in a 31 year old male patient at the diagnosis. We performed 32 seance phlebotomy during six months. At the end of the treatment we determined a limited improvement in hypogonadism and hepatic dysfunction whereas the progression in the diabetes. These findings showed that in advanced hemochromatosis system disorders except the pancreatic damage may response to the treatment.
