Özet
Feokromositomalardan katekolaminlerin yanısıra katekolamin yıkım ürünleri ve başka pek çok madde salgılamasına karşın günümüze kadar CRH salgısı olan sadece 3 vaka bildirilmiştir. Biz, burada ektopik CRH salgısı düşündüğümüz bir vakayı sunuyoruz. Kliniğimize başka bir merkezde sol sürrenal bezde 3 cm çapında kitle tesbit edilerek sevk edilen kadın hasta adrenal kitle etyolojisi araştırılmak üzere yatırıldı. Yatırıldığındaki fizik muayenesinde KB: 130/90 mmHg, N: 96/dk ritmik, sistem bulguları normal idi. Tam kan sayımı, tam idrar tetkiki, PA akciğer grafisi ve EKG normal idi. Plazma norepinefrin [813.5 pg/ml (N: 95-446)] ve idrar normetanefrin [541.3 (g/24 saat (N:88-444)] düzeyleri yüksek diğer katekolaminlerin düzeyleri normal idi. Bazal plazma kortizol düzeyi 20 (g/dl iken, deksametazon supresyon testlerinde baskılanma olmadı (1 mg: 20; 2 mg: 15; 8 mg: 19; 16 mg: 12 (g/dl). Bazal ve deksametazon supresyon testleri sırasındaki ACTH düzeyleri normal idi. Hipofiz MR normal bulundu. Yatışı süresince normotansif seyretti ve hipertansif atağı olmadı. Uygun hazırlık .sonrası sol sürrenalektomi yapıldı ve patolojik tanı feokromositoma geldi. Rezeksiyon materyalinde ACTH için immunohistokimyasal boyama menfi sonuç verdi. Post-op dönemde oral prednizolon tedavisi gerekli oldu ve post-op 9 aylık dönemde, ilaca uyum problemi nedeniyle, 3 kez IV steroid ve mayi tedavisi gerektiren sürrenal kriz atağı oldu. Bu ataklardan birinde ACTH düzeyi ölçülebilen ve yüksek bulundu. Biz dünya literatüründeki dördüncü ektopik CRH salgısı gösteren feokromositoma vakasını sunduk. Vakamız ve diğer iki rapordaki hastalardan bildirilen veriler feokromositoma vakalarının ektopik CRH sekresyonu yönünden dikkatli değerlendirilmesi ve araştırılması gerektiğini düşündürmektedir.
Abstract
Although several molecules, including catecholamine degradation products, are secreted from pheochromocytomas, only three cases of CRH secretion had been reported up today. Here we present a case of pheochromocytoma that is suggested to have CRH secretion. A female patient with a 3 cm mass in the left adrenal gland has been hospitalized in order to find the etiology of the mass. At the time of hospitalization her arterial blood pressure was 130/90 mmHg, with a pulse rate of 96/ min; physical examination was completely normal. Her complete blood count, routine urinalysis, posteroanterior chest X-ray, and electrocardiogram were normal. While plasma norepinephrine [813.5 pg/ml (N: 95-446)] and urinary normetanephrine [5413 (g/24 saat (N: 88-444)] levels were high, levels of other catecholamines were normal. Basal plasma cortisol level was 20 (g/dl, and did not suppress in response to dexamethasone suppression (1 mg: 20; 2 mg: 15; 8 mg: 19; 16 mg: 12 (g/dl). Serum ACTH levels were normal at basal and dexamethasone suppression. MR imaging of hypophysis was normal. She was normotensive and no attacks of hypertension were observed during the period of hospitalization. Left surrenalectomy is performed after appropriate preoperative measures. Histopathological diagnosis was pheochromocytoma. Immunohistochemical staining for ACTH was negative. At the postoperative period she needed oral prednisolone therapy and had 3 attacks of adrenal crisis requiring IV steroid and fluid therapy within 9 months due to incompliance to oral prednisolone. ACTH level was found to be high in one of these attacks. We presented the fourth case of ectopic CRH secretion from pheochromocytoma in the literature. We suggest that patients with pheochromocytoma should be carefully evaluated for the presence of ectopic CRH secretion.
