Tip 2 diabetes mellitus'ta metabolik regülasyon, diabet süresi ve lipidlerin diabetik retinopati gelişimi üzerine etkileri

Tip 2 diabetes mellitus (DM)'lu hastalarda diabetik retinopati (DR) gelişimi üzerinde metabolik regülasyonun, diabet süresinin ve lipidlerdeki değişimlerin etkilerini değerlendirmek amaçlandı. Elli kadın ve 26 erkek olmak üzere ortalama yaşı 59.26±10.3 (38-87 yaş arası) olan toplam 76 tip 2 DM'lu hasta çalışmaya alındı. On iki saatlik açlıktan sonra glukoz, HbA1c, total kolesterol, trigliserid, LDL ve HDL kolesterol tayini için kan örnekleri alındı. Tüm hastaların fundoskopik göz muayeneleri, DR tespiti ve sınıflandırılması için yapıldı. Hastalar plazma HbA1 c seviyelerine (%8) ve diabet sürelerine (0-5, 6-15, >15 yıl) göre üçer ayrı grup halinde sınıflandırıldı. Grupların karşılaştırılmasında Chi-square (x2) ve Student-t testi, korelasyon analizlerinde Spearman testi kullanıldı. Elli (%65.8) hastanın fundoskopik incelemesi normal bulunurken, 14 (% 18.4) hastada background DR ve 12 (%15.8) proliferatif DR tespit edildi. DR sıklığı ve tipi HbA1c %8 grubunda sırası ile %95 normal, %5 background DR; %60 normal, %10 background DR, %30 proliferatif DR; %54.3 normal, %26.2 background DR ve %19.5 proliferatif DR olarak tespit edildi. Plazma HbA1c seviyesinde artış ile DR sıklığının anlamlı olarak (x2=10.4, p15 yıl) göre hastalar gruplandığında DR görülme sıklığı sırası ile %91.6 normal, %4.2 background DR, %4.2 proliferatif DR; %61 normal, %27.8 background, %11.2 proliferatif DR; %37.5 normal, %18.7 background, %43.8 proliferatif DR tespit edildi. Abstract
We aimed to assess the effects of metabolic regulation, duration of diabetes and lipids on the development of diabetic retinopathy (DR) in type 2 diabetes mellitus (DM). Seventy six patients with type 2 DM (50 women, 26 men), mean age 59.26±10.3 were screened for fasting blood glucose, HbA1c, total cholesterol, triglyceride, LDL cholesterol, HDL cholesterol. Fundoscopic examination (FE) was performed for DR diagnosis and classification. Patients with or without DR were grouped according to their plasma HbA1c levels (8%) and to duration of diabetes (0-5, 6-15 and >16 years). Chi-squre (x2) and Student-t test for comparison and Spearman test for correlation analysis were used. Fifty (65.8%) patients had normal FE, 14 (18.4%) had background DR and 12 (15.8%) patients had proliferative DR. In the HbA1c 8% group 54.3% normal FE, 26.2% background DR, 19.5% proliferative DR was observed. With increase in HbA1c there was also increased DR incidence (x2=10.4, p16 years; 37.5% normal FE, 18.7% background, 43.8% proliferative DR respectively.
