Özet

Hipertrigliseridemi, akut pankreatitin nedenlerinden birisidir; sıklığı pankreatitli hastalarda %4-53 arasında değişmektedir. İki buçuk yıldır karın ağrısı şikayeti olan 36 yaşında bayan hasta karın ağrısı ile hastanemize başvurdu. Bir yıl önce trigliserid yüksekliği saptanmış ve pankreatite sekonder kist nedeni ile opere edilmiş olduğu öğrenildi. Tetkiklerinde trigliserid: 1665 mg/dL, amilaz: 323 U/L (0-90), lipaz: 1741 U/L (23-203), glukoz: 92 mg/dl, BUN: 14 mg/dL, kreatinin: 1.10 mg/dL, lökosit: 8200/mm3, Hb: 13.4 g/dL, karın BT ve MR'ında; konturlarında düzensizlik olan ödemli pankreas saptandı. Bu bulgulara göre; hipertrigliserideminin indüklediği kronik zeminde gelişen akut pankreatit tanısı kondu. Medikal tedavi ile semptomları ve laboratuvar bulguları birkaç hafta iç erişinde düzeldi. Hastaya diyet ve gemfibrozil önerilerek taburcu edildi. Sonuç olarak; karın ağrısı gibi nonspesifik semptomlar karşısında pankreatitin de düşünülmesi ve etyolojide hipertrigliserideminin akla gelmesi gerekmektedir.

Abstract

Hypertriglyceridemia is one of causes of acute panceratitis, its frequency in patients with pancreatitis varies between 4% and 53%. A thirtysix year-old woman was admitted to our hospital with the complaints of abdominal pain which had been present for about two and a half year. Her history was including high triglycerid levels and operation of a cyst secondary to pancreatitis one year before admission. Laboratory values were as follows: triglyceride: 1665 mg/dL, amylase: 323 U/L(0-90), lipase: 1741 U/L (23-203), glucose: 92 mg/dl, BUN: 14 mg/dL, creatinine: 1.10 mg/dL, leucocytes: 8200/mm3, hemoglobin: 13.4 g/dL, abdominal CT and MR showed irregularity of the shape of oedematous pancreas. Owing to these findings, diagnosis of acute execerbation of chronic pancreatitis was established which had been induced by hypertriglyceridemia. Symptoms and laboratory findings improved in a few weeks by the medical approach. She's been discharged with diet and gemfibrozil treatment. In conclusion; during the conditions such as abdominal pain as a nonspesific symptom, pancreatitis should be considered and hypertriglyceridemia should be remembered.
