Özet
Erkek yalancı hermafroditliği (EYH), gonadların testis olduğu ancak genital kanalların ve/veya dış genitalyanın yetersiz maskülizasyonu ile seyreden bir seks farklılaşma bozukluğudur. EYH, etiolojik faktörlere göre çeşitli alt tiplere ayrılmaktadır. On sekiz yaşında, fenotipik olarak kadın görünümündeki hasta, adet görememe şikayetiyle Kocaeli Üniversitesi Tıp Fakültesi Hastanesi Endokrinoloji polikliniğine başvurdu. Hastanın sekonder seks karakterleri 12-13 yaşında tam olarak gelişmişti. Fizik muayenede obezite mevcuttu. Normal aksiler kıllanma ve meme gelişimi gözlendi. Genital muayenede, hafif küteromegali saptandı. Himen 2 adet orifise sahipti ve 7-8 cm ilerlenebilen kör bir vajen mevcuttu. Testisler palpe edilemedi. Karyotip, 46 XY olarak tespit edildi. FSH ve LH düzeyleri artmıştı. Plazma testosteron, 17-OH progesteron ve androstenedion düzeyleri ise düşüktü. KCG uyarısıyla bu hormonlarda bir artış gözlenmedi. Genital USG ve MR'da genital yapılar izlenemedi. Hasta bu klinik ve laboratuar bulgularıyla Leydig hücre agenezi veya hipoplazisine bağlı yalancı erkek hermafroditliği tanısı aldı. Endokrinolog, psikiyatrisi, ürolog ve adli tıp uzmanlarından oluşan bir heyet tarafından değerlendirildi. Hastaya östrojen tedavisi başlanıldı ve bilateral kavernozektomi, bilateral inguinal testis ekzisyonu ve bilateral inguinal herniografi operasyonu uygulandı. Örneklerin patolojik olarak incelenmesi sonucu, germ hücre aplazisi gösteren testiküler doku ve nadir Leydig hücreleri saptandı. Erkek yalancı hermafroditliğinin nadir bir nedeni olan Leydig hücre hipoplazisinden, LH reseptör gen mutasyonları sorumlu tutulmaktadır. Bu olgu sunumunda ileri derecede Leydig hücre hipoplazisi olan erişkin bir yalancı erkek hermafrodit olgusu tartışılmıştır.
Abstract
Male pseudohermaphroditism is a condition of sex differention disorder in which the gonads are testes but the genital ducts and/or external genitalia are incompletely masculinized. Male pseudohermaphroditism is classified to subtypes according to etiological factors. Eighteen years old, phenotypically female patient was admitted to Kocaeli University Hospital, Endocrinology outpatient clinic with complaint of primary amonerrhea. Her secondary sex characters were well-developed at the age of 12-13 years. At physical examination she had obesity. Her axiller hair and breast development were normal. In genital examination, she had slight clitoral enlargement. Hymen had 2 orifices with blind, 7-8 cm vaginal pouch. No testes were palpated. Karyotype examination was 46 XY. FSH and LH levels were elevated. Plasma testosterone, 17-OH progesterone and androstenedione levels were low and they were failed to increase after hCG stimulation. Genital USG and MR could not able to show any genital structure. Patient was diagnosed as male pseudohermaphroditism due to Leydig cell agenesis or hypoplasia with these clinical and hormonal profile. Patient was evaluated by committee including endocrinologists, psychiatrist, urologist and physicians from forensic medicine. Estrogen was prescibed to patient and bilateral cavernosectomy, bilateral inguinal testis excision and bilateral inguinal herniography operation was performed. Pathology specimen was, testicular tissue with germ cell aplasia and decreased number of Leydig cells. Severe inactivating mutation of LH receptor have been recognized as the cause of Leydig cell hypoplasia, a rare form of male pseudohermaphroditism. An adult patient with Leydig cell hypoplasia as a cause of male pseudohermaphroditism was discussed in this case presentation.
