Özet

Tiroid cerrahisinde son dönemde minimal invaziv yaklaşım önem kazanmaktadır. Tekniğin uygulanabilmesi için en önemli kriterlerden biri bezin boyutlarının ve volümünün insizyona uygun olmasıdır. Ameliyat öncesi inceleme sıklıkla iki boyutlu ultrasonografi ile yapılmaktadır. Ancak bezin elipsoid yapısı bu yöntemle yapılan boyut ve volüm hesabının değerini kısıtlamaktadır. Bu çalışmada amaç iki boyutlu ultrasonografinin tiroid bezi boyutlarını değerlendirmedeki etkinliğini ortaya koymaktır. Ocak 2003 ile Aralık 2008 tarihleri arasında total tiroidektomi yapılan hastalar retrospektif olarak tarandı. Belirlenen zaman dilimi içerisinde ameliyat öncesi ultrasonografide ve spesmende tiroid lobu boyutları 3 eksende (uzunluk, genişlik ve derinlik) verilen hastalar çalışmaya dahil edildi, diğerleri çalışmadan çıkarıldı. Her iki lobun uzunluk, genişlik ve derinliğinin ultrasonografi (US) ile saptanan değerleri histopatolojik inceleme sırasında spesmenden yapılan ölçümlerle karşılaştırıldı. Her iki lobda, her üç eksende de ameliyat sonrası spesmenden elde edilen gerçek boyut ölçümleri, ameliyat öncesi US ile yapılan ölçümlere göre anlamlı derecede büyüktü. Literatürde video yardımlı tiroidektomiye uygun olan hastaların tüm tiroidektomi yapılacak olan hastalara oranı ancak %10 kadar olabilmektedir. Bu nedenle ameliyat öncesi değerlendirme çok önemlidir. Ancak burada da gösterildiği gibi iki boyutlu US tiroid bezinin boyutlarını değerlendirmede ciddi kısıtlamalara sahiptir.

Abstract

Minimally invasive surgery gains importance in thyroid surgery recently. One of the most important criteria; dimensions and volume of the gland should be compatible to the incision. Preoperative evaluations have been mostly done by two-dimension ultrasonography. But, ellipsoid structure of the gland restricts the evaluation by this method. The purpose of the study was to investigate the efficacy of the two-dimension ultrasonography in evaluation of the thyroid dimensions and volume. Patient records that performed total thyroidectomy between January 2003 and December 2008 were retrospectively examined. Only the patients whose records were complete for three axes (length, width, depth) were included in the study; the others were excluded. Both lobes' length, width and depth values detected by two-dimension ultrasonography and measured during histopathologic examination were compared. The values obtained from specimen during histopathologic examination were significantly higher than the values obtained by ultrasonography at preoperative evaluation. The ratio of the patients appropriate for the minimally invasive surgery is only 10% in the literature. Because of this, preoperative evaluation is quite important. However, two-dimension ultrasonography has serious restrictions in the evaluation of the thyroid dimensions as shown in this study.
