Özet

Tiroidektomi kabul edilebilir morbidite oranı ve çok düşük mortalite oranı ile çok fazla yapılan bir operasyondur. Total tiroidektomi bugün için tiroid hastalıklarının tedavisinde tercih edilen operasyon türüdür. Ancak birçok cerrah rekürren larengeal sinir hasarı ve postoperatif hipoparatiroidizm gelişme riskinden dolayı benign tiroid hastalıkları için total tiroidektomiyi tercih etmemektedir. Biz 2006-2009 yılları arasında Erzurum Numune ve Erzurum Bölge Eğitim Araştırma hastanesinde tiroid cerrahisi yaptığımız 411 hastayı inceledik. Hastaların histopatolojik karakteristikleri, operasyon tipleri, postoperatif hastanede kalma süreleri ve komplikasyonlar değerlendirildi. Hastaların ortalama yaşları 43.8±13.5 (15-78 yaş arası) ve kadın erkek oranı 6.2:1 idi. Preoperatif dönemde hastaların 369'u (%89.8) multinoduler guatr, 25'i (%6.1) soliter nodül ve 17'si (%4.1) graves hastalığı tanısı almıştı. Kalıcı bilateral rekürren larengeal sinir felç oranı %0, kalıcı unilateral rekürren larengeal sinir felç oranı %0, ancak geçici unilateral rekürren larengeal sinir felç oranı %1 idi. Kalıcı hipokalsemi %0.5, geçici hipokalsemi ise %7.3 oranında görüldü. Postoperatif mortalite %0 idi. Total tiroidektomi düşük morbidite oranı ile güvenilir bir yöntemdir. Ayrıca nükse sebep olmadığı ve kalıcı bir tedavi sağladığı için tiroid hastalıklarında cerrahi gerektiği zaman, total tiroidektomi yapılabilecek en iyi tedavi yöntemi gibi görünmektedir.

Abstract

Thyroidectomy is a common operation with very low mortality and an acceptable morbidity rate. Total thyroidectomy has become the predominant type of surgery used today for the treatment of thyroid diseases; however, many surgeons choose not to perform total thyroidectomy to treat benign thyroid diseases owing to the associated risk of postoperative hypoparathyroidism and recurrent laryngeal nerve damage. We retrospectively analyzed the records of 411 patients who underwent thyroid surgery for thyroid disease at the Erzurum Numune Hospital and Erzurum Region Research and Education Hospital between 2006-2009. Patient's histopathologic characteristics, operation types, postoperative hospital stays and complications were evaluated. The mean patient age was 43.8±13.5 years (range, 15-78) and the female to male ratio was 6.2:1. Diagnoses before surgery were multinodular goitre (n = 369, 89.8%), soliter nodular goitre (n = 25, 6.1%) and Graves disease (n = 17, 4.1%). The incidence of permanent bilateral recurrent laryngeal nerve palsy was 0 %and that of permanent unilateral recurrent laryngeal nerve palsy was 0%, whereas the incidence of temporary unilateral recurrent laryngeal nerve palsy was 1.0%. Permanent hypocalcemia occurred in 0.5 % and overall temporary hypocalcemia occurred in 7.3 %of patients. There was postoperative mortality was 0%. Total thyroidectomy is safe and is associated with a low incidence of disabilities. Furthermore, total thyroidectomy seems to be the optimal procedure, when surgery is indicated, for thyroid diseases, as total thyroidectomy has the advantages of immediate and permanent cure and no recurrences.
