Reoperatif tiroid cerrahisinde intraoperatif sinir monitörizasyonu: İlk sonuçlarımız

Özet
Amaç: İkincil tiroidektomilerde primer tiroid ameliyatlarına göre, postoperatif gelişen fibrozis ve skar dokusuna bağlı normal doku örüntüsünün kaybı nedeniyle kalıcı hipokalsemi ve rekürren larengeal sinirin yaralanma riski artmaktadır. Bu çalışmada intraoperatif sinir monitörizasyonunun rekürren paralizisini önlemedeki önemi araştırılmıştır. Gereç ve yöntem: Bu çalışmada Ocak 2005 ile Ekim 2010 tarihleri arasında kliniğimizde ilk ameliyatta bir lobda total lobektomiden daha sınırlı rezeksiyon yapılan nüks guatr veya lokal nüks nedeni ile reoperatif tiroid cerrahisi uygulanan tiroid kanserli hastaların verileri retrospektif olarak incelendi. Hastalar intraoperatif sinir monitörizasyonu yapılan (grup 1) ve yapılmayan (grup 2) olmak üzere iki gruba ayrıldı. Rekürren larengeal sinir paralizisi oranları karşılaştırıldı. Bulgular: Yaş ortalaması 49,5±11,8 olan 23 hastanın (19 kadın, 4 erkek) 6’sı grup 1’de, 17’si grup 2’de saptandı. Grup 1’deki 6 hastanın 4’üne çift taraflı, 2’sine tek taraflı girişim yapıldı. Postoperatif dönemde 1 hastada (%10, sinir sayısına göre) geçici paralizi saptandı. Rekürren sinir monitörizasyonu yapılmayan hastaların 12’sine çift taraflı, 5’ine ise tek taraflı girişim yapıldı. Postoperatif dönemde 2 hastada (%6,8, sinir sayısına göre) kalıcı vokal kord paralizisi saptandı. İki grup arasında geçici ve kalıcı vokal kord paralizisi açısından istatistiki olarak anlamlı fark saptanmadı (p>0,05, her iki karşılaştırma için). Sonuç: Bu çalışmadaki sınırlı sayıdaki sinir monitörizasyonlu olgular kliniğimizdeki reoperatif girişimlerdeki ilk sonuçlarımızı yansıtmaktadır. Gruplar arasında anlamlı fark saptanmamasına rağmen, monitörizasyon grubunda kalıcı sinir paralizisi olmaması, bu yöntemin reoperatif tiroid cerrahisinde kalıcı sinir hasarı riskini önlemede katkısı olabileceğini düşündürmektedir. Sinir monitörizasyonunun öncelikle reoperatif tiroid cerrahisinde seçilmesi gereken yardımcı bir yöntem olduğu kanısındayız.
Abstract
Aim: Due to postoperative fibrosis and loss of normal tissue pattern caused by scar tissue, the risks of permanent hypocalcemia and recurrent laryngeal nerve injury increase in secondary thyroidectomies as compared with primary thyroid operations. In this study, the importance of nerve monitoring in the prevention of recurrent nerve paralysis was investigated. Methods: The records of patients who had undergone limited resection of a lobe rather than total lobectomy in their first operation and who underwent reoperative thyroid surgery between January 2005 and October 2010 because of recurrence of goitre or local recurrence, were reviewed retrospectively. The patients were divided into two groups, as those who were subjected to nerve monitoring (group 1), and those who were not (group 2). Recurrent laryngeal nerve palsy rates were compared in the two groups. Results: The mean age of the 23 patients was 49.5±11.8 years. There were 6 patients in group 1 and 17 patients in group 2. In group 1, 4 patients had undergone bilateral surgical intervention, while unilateral intervention.had been performed in 2 patients. One patient (10% by number of nerves) had suffered from a temporary paralysis postoperatively. In the group without recurrent nerve monitoring, 12 patients had undergone bilateral intervention and 5 had undergone a unilateral intervention. Permanent vocal cord paralysis had developed in 2 patients (6.8% by number of nerve) postoperatively. There was no significant difference between the two groups in terms of temporary or permanent vocal cord paralysis (p >0.05). Conclusions: The limited number of patients with nerve monitoring in this study reflect the first results of reoperative interventions in our clinic. Although there was no significant difference between the groups, the absence of any permanent nerve damage in the monitored group suggesta that nerve monitoring in reoperative thyroid surgery contributes to prevention of risk of permanent nerve damage. We believe that the nerve monitoring procedure must be selected primarily for recurrent thyroid surgery as an auxiliary method.
