Özet

Amaç: Tiroid bezi hastalıklarında cerrahi tedavi, özellikle endemik bölgelerde düşük morbidite ve mortalite oranları ile sıkça uygulanan bir tedavi yöntemidir. Bu çalışmadaki amaç tiroid bezi hastalıklarının endemik olarak sık görüldüğü bölgelerde uygulanan cerrahi tedavi yöntemlerini ve bunlara bağlı gelişen morbidite oranlarını incelemek ve sonuçları sunmaktır. Materyal ve metot: Temmuz 2007 ile Temmuz 2011 tarihleri arasında Düzce Atatürk Devlet Hastanesi Genel Cerrahi Kliniğine başvuran ve tiroid bezi hastalıkları tanısıyla ameliyat edilen toplam 772 hasta retrospektif olarak incelenmiş ve tedavi sonrası sonuçlar sunulmuştur. Bulgular: Hastaların 670 tanesi (%86,8) kadın, 102 tanesi (%13,2) erkekti. Yaş ortalaması 44,3 idi. Hastaların 536 tanesine (%69,4) bilateral total tiroidektomi, 159 tanesine (%20,6) bilateral near total tiroidektomi, 17 tanesine (%2,2) bilateral subtotal tiroidektomi, 10 tanesine (%1,3) tek taraf total tek taraf subtotal tiroidektomi, 1 tanesine (%0,1) tek taraf total tek taraf near total tiroidektomi, 49 tanesine (%6,4) tek taraf total tiroidektomi yapıldı. Ameliyat edilen hastaların 85 tanesinde (%11) komplikasyon gelişirken, 687 tanesinde (%89) komplikasyon gelişmedi. Ameliyatta çıkarılan dokuların histopatolojik incelemesi sonucunda 107 hastada (%13,9) malign patoloji, 665 hastada (%86,1) benign patoloji saptandı. Sonuç: Tiroid bezi hastalıkları için kliniğimizde uygulanan cerrahi tedavi yöntemlerinin komplikasyon oranları literatür bilgileri ile uyumluluk göstermektedir.

Abstract

Aim: Surgical therapy is frequently used for the treatment of thyroid gland diseases, particularly in the endemic regions, with low morbidity and mortality rates. The aim of the present study is to investigate the surgical therapies and associated morbidity rates in the regions which are endemic for thyroid gland diseases and to present the results. Material and method: A total of 772 patients, who presented to General Surgery Clinic of Düzce Atatürk State Hospital between July 2007 and July 2011 and underwent surgical procedure due to thyroid gland disease, were retrospectively investigated and postoperative results were presented. Results: Six hundred and seventy of the patients (86,8%) were female and 102 (13,2%) were male. The mean age was 44,3 years. Of the patients, 536 (69,4%) underwent bilateral total thyroidectomy, 159 (20,6%) underwent bilateral nearly-total thyroidectomy, 17 (2,2%) underwent bilateral subtotal thyroidectomy, 10 (1,3%) underwent unilateral total unilateral subtotal thyroidectomy, 1 (0,1%) underwent unilateral total unilateral nearly-total thyroidectomy, and 49 (6,4%) underwent unilateral total thyroidectomy. While 85 (11%) of the patients that underwent surgical procedure developed complications, no complication was detected in 687 (89%). Histopathological examination of surgical materials revealed malignancy in 107 (13,9%) and benign pathology in 665 (86,1%) patients. Conclusion: The complication rates of surgical procedures performed in our clinic for thyroid gland diseases show consistency with the literature.
