Özet

Hipertiroidizm tedavisinde antitiroid ilaçlar, radyoaktif iyot ve cerrahi tedavi gibi alışılagelmiş klasik yöntemler zaman zaman yetersiz kalabilmektedir. Tiroid arter embolizasyonu klasik yöntemlerle tedavi edilemeyen seçilmiş olgularda etkin ve güvenli bir tedavi alternatifi olabilir. Baş-boyun bölge metastatik malign tümörü nedeniyle opere edilmiş, radyoterapi almış ve kemoterapi almakta olan 32 yaşında erkek hastaya Graves hastalığı tanısıyla farmakoterapi başlandı. İlaç yan etkisi nedeniyle farmakoterapi devam ettirilemeyen, iyot maruziyeti nedeniyle radyoiyot tedavisi uygulanamayan ve cerrahisi riskli olup başarı şansı düşük olan hastaya selektif tiroid arter embolizasyonu uygulandı. İşlem sonrası ciddi komplikasyon gelişmedi, takipte hasta medikasyon almaksızın ötiroid seyretti. Takibin 11. ayında nüks hipertiroidizm gözlenen hastaya radyoiyot tedavisi verildi. Tiroid arter embolizasyonu düşük invazivitesi ve etkinliği nedeniyle Graves hastalığının zor klinik durumlarında terapötik bir seçenek olarak kullanılabilir.

Abstract

The traditional treatment methods of hyperthyroidism like antithyroid drugs, radioactive iodine and surgery may be insufficient sometimes. Thyroid artery embolization may be an effective and safe therapy choice in selected cases who can not be treated with conventional therapies. Pharmaco therapy was given to the 32-year- old male patient with Graves disease diagnosis who was operated for metastatic malignant tumour of head-neck region, took radiotherapy and receiving chemotherapy. Selective thyroid artery embolization was performed to the patient who could not continue pharmacotherapy due to drug adverse effects, who can not take radioiodine therapy because of iodine exposure and who can not be operated due to high risk and low chance of success. No serious complication was observed after procedure, and the patient remained euthyroid without medication. Radioiodine therapy was given to the patient because of recurrent hyperthyroidism in the eleventh month of follow-ups. Thyroid artery embolization can be used as a therapeutic option in challenging clinical situations of Graves disease because of its low invasivity and high effectiveness.
