Tekrarlayan guatrda cerrahi tedavi

Özet

Amaç: Tiroid cerrahisi sık uygulanan cerrahilerdendir ve tüm guatr operasyonlarından sonra %10-15 hastaya tekrar cerrahi girişim gerekmektedir. Bu çalışmada tekrarlayan guatr sebebiyle opere edilen hastalara ait sonuçlar literatür bilgileri eşliğinde değerlendirilmiştir. Gereç ve yöntem: Hasta dosyalarından retrospektif olarak inceleme yapılarak hasta bilgileri, operasyon bilgileri, operasyon sonrası komplikasyonlar kayıt edildi.Sonuçlar: On üç hasta tekrarlayan guatr sebebiyle opere edildi. Hastaların tümü bayan idi. Dört (%31) hastada geçici hipokalsemi görüldü. Başka komplikasyon görülmedi. Tartışma: Tekrarlayan guatrı önlemek için ilk operasyonda total tiroidektomi uygulanmalıdır. Tekrarlayan guatr cerrahisinde komplikasyonlar ilk cerrahiye göre yüksek olmakla birlikte deneyimli cerrahlar tarafından düşük morbidite ile yapılabilir.

Abstract

Introduction: Among surgical procedures, thyroidectomy is commonly performed surgery. Approximately 10-15 %of patients require re-thyroidectomy after first goitre operations. In this article we analyze results of patients undergone thyroidectomy for recurrent goitre. Material and method:Records of patients that have un- dergone thyroidectomy for recurrent goitre are investigated retrospectively. Results: Thirteen patients operated for recurrent goitre. All of them was female. Transient hypocalcemia was developed 4 (31%) patients. Additional complications were not observed. Discussion: For prevention of recurrent goitre, first operation must be total thyroidectomy. Although complications are higher in recurrent surgery when compared with first operation, this operation can be performed with lower morbidity rates by experienced surgeons.
