Tip 2 Diyabetli Bireyin Gastrik Bypass Sonrası Evde Bakımının Tedaviye Uyumuna Yansıması: 6 Aylık İzlem

Özet
Kronik hastalığı olan bireyin bakım gereksinimlerinin karşılanmasında sunulan hemşirelik hizmetleri önemlidir. Olgunun, 23 yıldır tip 2 diyabeti vardır, komorbiditelerinin artması nedeniyle gastrik bypass ameliyatı yapılmıştır. Hastalığın yönetimini sağlamakta güçlük çeken bireyin ameliyat sonrası evde bakım sürecinde, holistik bir yaklaşımla hemşirelik bakımı gerçekleştirilmelidir. Bu nedenle olgu sunumu bireyin gastrik bypass sonrası Fonksiyonel Sağlık Örüntüleri Modeli ve NANDA sınıflamasına göre evde bakım sürecini değerlendirmek amacıyla yapılmıştır. Olgu ile ameliyattan sonra ilk izlemin ardından, 3. ve 6. ayda görüşmeler gerçekleştirilmiştir. İzlem sürecinde bireyin tedaviye uyum sağladığı, glisemik kontrolün sağlandığı, kilo verdiği, bel/kalça oranını azaldığı, lipit seviyelerinde iyileşme olduğu gözlenmiştir. Sonuç olarak; hemşire tarafından gerçekleştirilen 6 aylık izlemin bireyin tedaviye uyum sürecine katkı sağladığı ve istendik yaşam davranışlarını sergilemesini kolaylaştırdığı görülmüştür.
Abstract
Nursing services are important in addressing the care needs of individuals with chronic disease. The patient with the 23-year history of type 2 diabetes underwent gastric bypass surgery due to increased comorbidities. The post-operative home care of the patient having difficulty in managing her diabetes should included nursing services with a holistic approach. Thus, this case report is created to evaluate the home care process of the post-gastric-bypass individual as per the Functional Health Patterns Model and the NANDA classification. Following the first follow-up after the surgery, the case was evaluated at the 3rd and 6th months. It was observed in the follow-ups that the case adhered to the treatment plan, with improved glycemic control and lipid levels as well as decreased waist/hip ratio. This study concluded that the 6-month follow-up by a nurse contributes to the patient’s adherence to the treatment and encourages the patient to demonstrate the healthy life behaviors targeted by the treatment.
