Ortopedi Hastalarının Venöz Tromboemboli Riskine İlişkin Bilgi ve Uygulamaları

Özet
Amaç: Çalışma ortopedi ameliyatı geçiren hastaların venöz tromboemboli (VTE) riskini önlemeye yönelik bilgigereksinimleri ve uygulamalarının belirlenmesi amacı ile tanımlayıcı olarak yapılmıştır.Gereç ve Yöntemler: Çalışma Ankara’daki bir eğitim araştırma hastanesinde 18.07.2018- 19.10. 2018 tarihleriarasında 140 hasta ile yapılmıştır. Verilerin toplanması amacı ile araştırmacı tarafından oluşturan hasta tanımlayıcıbilgiler formu, VTE hasta bilgisi formu, Derin Ven Trombozu (DVT) tanılama kriterleri formu, Autar Risk TanılamaÖlçeği kullanılmıştır. Verilerin analizinde sayı ve yüzde gibi tanımlayıcı istatistikler kullanılmıştır.Bulgular: Çalışmamızda Autar DVT Riski Tanılama Ölçeğine göre ameliyat sonrası dönemde hastaların çoğundabirden fazla VTE risk faktörü olduğu, en fazla ileri yaş, normal kilonun üzerinde olma, ortopedik (bel altı) cerrahigirişim geçirme ve oldukça bağımlı (yardıma ihtiyacı var) olma risk faktörlerine sahip oldukları saptanmıştır. Çalışmayakatılan hastaların %81,4’ünün VTE tanımını, %87,8’inin VTE risk faktörlerini, %87,8’inin VTE riskini azaltacakuygulamaları bilmediği saptanmıştır. Ameliyat sonrası dönemde tromboprofilaksi yöntemlerinin uygulanmasına rağmenhastaların yarıdan fazlasının kompresyon çorabı kullanımı hakkında ve %80,9 oranında antikoagülan ilaç kullanımıhakkında bilgilerinin olmadığı belirlenmiştir.Sonuç: Çalışmamızda hastaların VTE açısından birden fazla risk faktörüne sahip olmalarına karşın yüksek oranlardabilgi eksiklikleri olduğu saptanmıştır. Hastaların VTE ile ilgili bilgi düzeylerinin belirlenmesi ve bu konudakibilgilerinin eğitimlerle artırılması; hastaların bireysel değerlendirme yapmalarını sağlayacak, hastanede ve taburculuksonrası dönemlerde VTE görülme sıklığı azaltacaktır. Bunun sonucunda hastaların yaşam kalitesi artacak, hastanedekalış süreleri ve sağlık harcamalarının azalacağı düşünülmektedir.
Abstract
Aim: The study was conducted as a descriptive study in order to determine the knowledge requirements and practices to prevent the risk of venous thromboembolism in patients undergoing orthopedic surgery. Material and Methods: The study was conducted with 140 patients between 18. 07. 2018- 19.10. 2018 in an education research hospital in Ankara. The patient descriptive information form, VTE patient information form, DVT diagnostic criteria form which is prepared by the researcher and Autar Risk Diagnosis Scale were used in order to collect the data. Descriptive statistics such as number and percentage were used in the analysis of the data Results: In our study, according to the Autar DVT Risk Diagnosis Scale, most of the patients had more than one risk factor in the postoperative period, the most experienced risk factors were having advanced age, being above normal weight, having orthopedic (under waist) surgical intervention and living highly dependent (help requirement) risk factors. Among the patients participated in the study, 81.4% had not known VTE definition, 87.8% VTE risk factors, 87.8% the applications to reduce the risk of VTE. It was found that more than half of the patients did not have information about the use of compression stockings and 80.9% of the patients did not have any information about the use of anticoagulants despite the high rates of anticoagulant drug use. Conclusion: In our study, it was found that although the patients had more than one risk factor for VTE, there was a high level of information deficiency. Determining the level of knowledge of patients about VTE and increasing their knowledge on this subject with trainings will allow them to perform individual evaluations and thus reduce the incidence of VTE in hospital and post-discharge periods. As a result of this, it is considered that patients’ quality of life will increase and hospital stay and health expenses will decrease.
