Jejunal atrezi tedavisinde farklı cerrahi yöntemlerin karşılaştırılması

Özet

Bu retrospektif çalışmada 1989-1999 yılları arasında proksimal jejunal atrezi nedeni ile opere edilen 21 yenidoğanda uygulanan cerrahi yöntemlerin sonuçları değerlendirilmiştir. Olguların 12'si erkek, 9'u kız, ortalama doğum ağırlıkları 2450±175 (2200-2700) gr. dır. Olguların 5'inde yan-yana anastomoz, 9'unda daraltma ("tapering") sonrası uç-uca anastomoz, 7'sinde ise yüksek jejunal rezeksiyon sonrası uç-yan (oblik) anastomoz uygulanmıştır. Anastomoz kaçağı, yeniden operasyon gereksinimi, tam enteral beslenmeye geçiş süresi ve mortalite oranlarının yüksek jejunal rezeksiyon+uç-yan anastomoz uygulanan grupta diğer gruplara göre belirgin oranda düşük olduğu görülmüştür. Proksimal jejunal atrezi tedavisinde uygun vakalarda yüksek jejunal rezeksiyon+uç-yan anastomozun yan-yana anastomoz ve "tapering"+uç-uca anastomoz tekniklerinden sonuçları itibariyle daha başarılı bir yöntem olduğu kanaatına varılmıştır.

Abstract

In this retrospective study, we evaluated the results of different surgical techniques which were used in the treatment of 21 infants with proximal jejunal atresia between 1989 to 1999. There were 12 male and 9 female infants. Median birth weight was 1450±175 gr. Five infants were treated with side-to-side anostomosis, nine with antimesenteric tapering jejunoplasty and end-to-end anostomosis, and seven with high jejunal resection and end-to-side oblique anostomosis. We observed that the percentage of anastomotic leak, reoperation rate, full fed time and mortality rate were lower in the infants who were operated with high jejunal resection and end-to-side oblique anostomosis than the infants who were treated with other techniques. We think that the results of the end-to-side oblique anastomosis with wide jejunal resection is superior than side-to-side anostomosis and anti-mesentering tapering jejunoplasty in infants with jejunal atresia.
