İsradipinin antihipertansif etkinliği ve bazı böbrek fonksiyon göstergelerine etkisi ve yan etki profili

Özet
İsradipin, hafif ve orta derecede hipertansiyonda kullanılan, dihidropiridin grubu yeni bir kalsiyum antagonistidir. Bu çalışmada, hafif ve orta derecede primer hipertansiyonlu 18 hastada 12 hafta süre ile günde tek doz, oral 5 mg yavaş salınımlı isradipin kullanıldı. İsradipinin; antihipertansif etkinliğinin yanında glomerular filtrasyon hızına (GFR), renal sodyum ekskresyonuna etkisi ve isradipin kullanımı sırasında görülen yan etki profili araştırıldı. Sonuçta, belirgin antihipertansif etkileri dışında, glomerul filtrasyon hızında ve 24 saatlik idrarda sodyum atılımında istatistiksel olarak anlamlı olmayan artışlar saptandı. Üç ay süre ile isradipin kullanan hastalarda İsradipinin kalp ritmi üzerine olumsuz bir etkisi saptanmadı. İlacın kullanıldığı süre içinde %12 oranında ilacı kesmeyi gerektirecek flushing, baş ağrısı, baş dönmesi, çarpıntı görüldü ve bu yan etkinin diğer dihidropiridin kalsiyum antagonistlerinde de olduğu gibi aşırı vasodilatasyona bağlı olarak geliştiği düşünüldü. Bunun dışında önemli bir yan etki görülmedi.
Abstract
İsradipine is a calcium antagonist which is in the dihyropyridine group and it is used for mild to moderate hypertension. In this study we investigated the antihypertensive effects of a slow releasing drug; isradipine; which was given for 12 weeks, 5mg daily per oral. Also the effects on glomerular filtration rate (GFR), renal sodium excretion and the side effects of isradipine were investigated. In conclusion, isradipine showed remarkable antihypertensive effects. In addition, isradipine increased the glomerular filtration rate and 24 hour - Na excretion rate, but these results were not statistically significant. During the 3 months study period, isradipine showed no adverse effects on cardiac rhytm. In 12% of the study group side effects like flushing, headache, and palpitation led to termination of the therapy. These side effects were thought to be due to the excessive vasodilatation which can also be seen with the other calcium-channel-blocking-agents. No other important side effects have been observed.
