Oral kavite tümörlerinde lenfatik metastazın anatomik lenfatik drenajla ilişkisi

Özet
Oral kavitenin normal anatomik lenfatik drenajı farklı bölgeleredir. Bu çalışmada amaç, oral kavite tümörlerinin lenf nodu metastaz profilleri ile normal anatomik drenaj arasındaki korelasyonu araştırmak ve bu olgularda servikal lenf nodlarına yönelik fizik muayene, radyolojik ve histopatolojik incelemelere ışık tutmaktır. Bu amaçla, Dokuz Eylül Üniversitesi Tıp Fakültesi Patoloji Anabilim Dalı arşivinde bulunan, 1987-96 yıllarına ait, 24 oral kavite tümör olgusunun standart boyun diseksiyon materyalleri gözden geçirilmiştir. Bu olgulara ait biopsi raporlarında lenf nodu metastaz varlığı ve anatomik dağılımları araştırılmış, bu dağılımlar normal lenfatik drenaj dağılımları ile karşılaştırılmıştır. Olguların 10' u (%41) dil, 6' sı (%25) dudak, 3' ü (%12) retromolar frigon, 3' ü (%12) ağız tabanı, 1' i (%S) sert damak ve 1' i (%5) bukkal mukoza yerleşimlidir. Metastaz izlenen toplam 16 (%66) olgu arasında, retromolar trigon ve dil tümörlerinde lenf nodu metastazının sırasıyla %100 ve %90 olduğu görülmektedir. Dil tümörleri genelde üst derin servikal ve üst servikal lenf nodlarına metastaz yaparken, retromolar trigon tümörlerinde farklı bölgelere olan bir metastaz paterni bulunmaktadır. Sonuç olarak bu çalışmada metastatik olguların normal anatomik lenfatik drenajları ile uyumlu bir metastaz paterni gösterdiği ve bu nedenle boyun diseksiyon materyallerinin, standart seviyelere göre ayrılarak incelenmesinin anatomik lenfatik drenajla bağlantılı olduğu düşünülmüştür.
Abstract
The lymphatic drainage of oral cavity includes different cervical lymph nodes.The goal of this study is to investigate the correlation between normal anatomic lymphatic drainage and nodal involvement profiles of cancers arisingfrom different anatomic regions of oral cavity and if this correlation might guide the physical, radiological and histopathological examination. The present study includes 24 oral cavity tumors diagnosed at Dokuz Eylül University Hospital, Department of Pathology between 1987-1996. The presence of lymphatic metastasis and its anatomic distribution in standard cervical dissection materials, and its correlation with normal anatomic lymphatic drainage were investigated. Of the 24 cases, ten (41%) were located at the tongue, six (%25) at the lips, three (12%) at the retromolar trigon, three (12%) at the floor of the mouth, one (5%) at the palate and one (5%) at the buccal mucosa. Among the overall 16 metastatic cases (66%), lymph node involvement was apparent at the tumors of retromolar trigon and tongue with a ratio of 100% and 90%, respectively. The metastatic route at the cancers of the tongue included generally upper cervical and upper deep cervical lymph nodes, however it was insignificant at the retromolar trigone. To conclude, the metastatic lymph node profiles of oral cavity cancers show good correlation with normal anatomic lymphatic drainage and in any examination aiming to detect nodal involvement, the lymphatic drainage system must betaken into account.
