Özet
Hipertrofik kardiyomiyopati (HK) otozomal dominant geçiş gösteren genetik bir hastalıktır ve toplumda %0.01-0.1 oranında görülmektedir. Bütün ırklarda tanımlanmış olmakla beraber farklı ırk gruplarında HK' nin prognozu ve ventrikülün tutulum biçimi değişkenlik gösterebilir. Japonlarda apikal tutulum daha sık görülürken, Yunanlılarda hipertrofinin şiddeti ve ani ölüm sıklığı Kuzey Avrupa ve Kuzey Amerikalılara göre daha azdır. Olguların yarısında ailevi geçiş olduğu, geri kalanlarda ise sporadik olarak görüldüğü saptanmıştır. Hipertrofinin derecesi, dağılımı, hastalığın başlangıç yaşı, klinik belirtilerinin tipi ve ciddiyeti de değişkendir. Belirgin kardiyak patolojiyi miyozit hipertrofisi ve sarkomer düzensizliği oluşturmaktadır. Bu yazıda ailevi geçiş gösteren hipertrofik kardiyomiyopati olguları sunulmuştur.
Abstract
Hypertrophic cardiomyopathy (HCM) is a genetic disease inherited as otosomal dominant, with frequency of %0,01 -0,1.There are several studies which describes HCM in different races in whom, prognosis and the type of ventricular involvement have shown some alterations. Apical involvement is more frequent in the Japanese people where as the severity of hypertrophy and frequency of sudden death in Greece is less when compared with North Europe and North America In the half of the cases, familial inheritance is seen, however the disease is sporadically present for the rest. The severity and the distribution of the hypertrophy, the starting age, and the severity of clinical manifestations are variable. Major pathology consists of the inflammation and hypertrophy of cardiac muscles and sarkomer irregularity. In this review, familial hypertrophic cardiomyopathy cases were presented.
