Özet

Sekiz yıldır tedaviye dirençli hipertansiyonu olan ve hipopotasemisi saptanan hastaya plazma aldosteron konsantrasyonu / plazma renin aktivitesi oranı yüksek bulunarak primer aldosteronizm tanısı konuldu. Sürrenal bilgisayarlı tomografi incelemesi normal olduğundan İdiyopatik hiperaldosteronizm düşünülerek spironolakton 200 mg/gün tedavisi başlandı. Dört hafta sonra 400 mg/gün dozuna çıkıldıktan sonra hastanın kan basıncı ve serum potasyum düzeyi normale döndü. Hasta halen spironolakton 200 mg/gün tedavisi ile takip edilmektedir. Spontan veya provake hipopotasemisi olan, standart tedaviye dirençli hipertansiflerde primer aldosteronizmden şüphelenilmeli ve plazma aldosteron konsantrasyonu /plazma renin aktivitesi oranı ile tarama yapılmalıdır.

Abstract

A 44 years old female patient with an 8 years history of hypertension being resistant to therapy presented with hypokalemia. Primary aldosteronism was diagnosed due to a high plasma aldosteron concentration / plasma renin activity ratio. Idiopathic hyperaldosteronism was considered due to normal findings on adrenal computer tomography and spironolactone 200 mg/day was started. Blood pressure and serum potassium level returned to normal after increasing spironolactone dose to 400 mg/day after 4 weeks. The patient is still followed with 200 mg/day spironolactone therapy- Primary aldosteronism should be suspected in hypertensive patients presenting with spontaneous or provoked hypokalemia and poor response to standart therapy and plasma aldosteron concentration / plasma renin activity ratio should be investigated.
