Özet

Altı yaşında çocuk hastada(B.A.) Malign Lenfoma, diffüz büyük, çentiksiz hücreli tip tanısı ile başlanan Vincristine, Cyclophosphamide Prednisolone, Daunorubicene, Diazomid, Allopurinol tedavisinin beşinci gününde böbrek fonksiyon bozuklukları gelişti. Olgu; klinik, histopatolojik, immunoenzimatik, elektron mikroskopik yöntemler ile değerlendirildi ve mezengioproliferatif glomerülonefrit (granüler Ig M,A birikimi gösteren) ve tubulointerstisyel nefropati olarak tanımlandı. Olguda gelişen patolojinin birden fazla etkene bağlı olduğu ve malign lenf om aya ikincil atipik lenfosit antijenleri ve/veya maligniteyle beraber olan sekonder enfeksiyonların başlatıcısı olduğu otoantikor yapımı ile karekterize immunkompleks nefriti ile beraber antineoplastik tedaviye bağlı hiperürisemik nefrotoksisitenin rol oynadığı düşünülmektedir. Bu olgu nedeniyle böbrek de lenfoma tutulumu olmaksızın gelişen immunkompleks nefriti ve tubulointerstisyel zedelenmenin patogenezinde; atipik lenfosit antijenleri, antineoplastik tedavi ve hiperürisemiye ikincil nefrotoksisitenin rolü irdelenmiştir.

Abstract

A six year-old child patient diagnosed as malign lymphoma of diffuse, large noncleaved cell type was treated by Vincristine, Cyclophosphamide, Prednisolone, Daunorubicine, Diazomid, Allopurinol. During the fifth day of treatment renal functional impairment were encountered. The case was analyzed clinically, histopathologically, immunoenzymatically electron microscopically and defined as mesangioproliferative glomerulonephritis (showing deposition of IgM, A) and tubulointerstitial nephropathy. In this case, the role of atypical lymphocyte antigens, nephrotoxicity secondary to hyperuricemia and antineoplastic treatment discussed in the pathogenesis of immuncomplex nephritis and tubulointerstitial damage occurring in lymphoma without renal involvement.
