Özet

AMAÇ: Son yıllarda diz patolojilerinin tanısında invasiv olmayan bir yöntem olan manyetik rezonans görüntüleme ( MRG ) artan bir sıklıkla kullanılmaktadır. Bu çalışmanın amacı artroskopi altın standart kabul edilerek, diz eklemi patolojilerinin tanısında MRG'nin yerini ortaya koymaktır. MATERYAL ve METOD: S.B. Haseki Hastanesi Ortopedi ve Travmatoloji Kliniğine 1995 - 97 yılları arasında başvuran 51 hastanın 53 dizine MRG tetkiki uygulandı. Daha sonra aynı dizlere artroskopi uygulayarak MRG'nin menisküs ve ön çapraz bağ lezyonlarındaki doğruluk, duyarlılık, özgüllük, negatif tahmini değerlendirme ve pozitif tahmini değerlendirme oranları saptandı. BULGULAR: MRG'nin menisküs lezyonlarını saptamadaki doğruluğu % 77, duyarlılığı % 66, özgüllüğü % 70.5, negatif tahmini değerlendirmesi % 92, pozitif tahmini değerlendirmesi % 57.5 olarak hesaplandı. MRG'nin ACL için doğruluk % 88, duyarlılık % 67, özgüllük % 93, negatif tahmini değerlendirmesi % 93, pozitif tahmini değerlendirmesi % 67 olarak bulundu. SONUÇ: Son yıllarda gelişmiş cihazların devreye girmesi yanında MRG'yi değerlendiren radyologların görgü ve deneyimlerinin artması ile gerçek (+) sonuç oranları giderek yükselmektedir. Ayrıca negatif tahmini değerlendirme oranı da büyük önem taşımaktadır. Bu parametre MRG'de herhangi bir patoloji saptanmayan hastaların yüzde kaçında artroskopik olarak lezyon olmadığını ortaya koyar. Çalışmamızda bu değer % 92 olarak bulunmuştur. Bu parametrenin anlamlı bulunması nedeniyle, herhangi bir nedenle artroskopi uygulanamayan olgularda MRG değerli bir tanı yöntemidir.

Abstract

Aim: Magnetic resonance imaging (MRI) has been used with an increasing frequency as a non-invasive method in the diagnosis of knee pathologies in recent years. The aim of this study is to reveal the value of MRI in the diagnosis of pathologies of the knee joint while the arthroscopy is accepted as the gold standard. Material & Method: Between 1995-1997 the patients who had complaints of the knee joint were evaluated with Magnetic Resonance Imaging, prior to arthroscopic examinations at Haseki Teaching Hospital, Department of Orthopaedics and Traumatology - Istanbul. Magnetic Resonance Images were obtained in a series of 53 knees of 51 patients. MRI studies were performed in two different centers and all of the arthroscopic procedures were performed by the same surgeons. The accuracy, sensitivity, specifity, negative predictive value and positive predictive value of MRI in the evaluation of medial and lateral menisci and anterior cruciate ligament lesions were determined. We observed that MRI offers a high percentage of accurate diagnoses in lesions of these structures. Results: The accuracy of MRI in detecting the lesions of menisci was calculated as 77 %. Other results were as follows : sensitivity 66 %, specifity 70.5 %, negative predictive value 92 %, positive predictive value 57.5 %. The value of MRI for the pathologies of ACL was found as follows : accuracy 88 %, sensitivity 67 %, specifity 93 %, negative predictive value 93 %, positive predictive value 67 %. Conclusion: Due to the use of highly developed equipment and the increase in the experience of radiologists evaluating MR Images, the rate of true positive results has been increasing in the recent years. Additionally, negative predictive value does also have a great importance. This parameter implicates the percentage of patients having no lesion arthroscopically, in patients in whom no pathology was detected with MRI. Negative predictive value was found as 92 % in our study. Since this parameter was found as significant, MRI was accepted as an excellent substitute for diagnostic arthroscopy especially in the patients who can not tolerate arthroscopic examination for any reason.
