Özet

AMAÇ: Yüksek enerjili ateşli silah yaralanmaları sonucu oluşan dirsek kondiler bölge Gustillo-Anderson Tip III kırıkları, anatomik özellikleri, damar ve sinir komşulukları, yumuşak doku sorunları, çoğunun parçalı olması gibi nedenlerle tedavileri güç ve morbiditeleri yüksek kırıklardır. Bu çalışmamızın amacı, bu tip kırıklardaki akut dönem tedavi yaklaşımlarımızı sunmaktır. YÖNTEM: Aralık 1993-Aralık 1997 tarihleri arasında GATA Ortopedi ve Travmatoloji A.D.'a yüksek enerjili ateşli silah yaralanması nedeniyle ilk 6-8 saat içinde getirilen ve tedavi edilen Tip III açık dirsek kondiler bölge kırığı olan 19 olgu değerlendirildi. Beşi Tip IIIa, 12'si Tip IIIb, 2'si Tip IIIc idi. Hepsine acil debritman, primer yumuşak doku kapatılması ve kırık tesbiti uygulandı. Tip IIIc kırıklarda vasküler tamir yapıldı. Tip IIIa kırıkların tümü ile Tip IIIb 5 kırıkta internal fiksasyon, Tip IIIb 7 kırıkta ve Tip IIIc kırıkların hepsinde eksternal fiksasyon uygulandı. SONUÇLAR: Ortalama takip süresi 32 ay (24.5-55 ay) olup, Morrey kriterlerine göre 5 olgu (% 29.5) iyi, 12 olgu (% 70.5) kötü sonuç alındı. Damar tamiri yapılan 2 olguda gelişen nekroz nedeniyle önkol amputasyonu uygulandı. Takip periyodunda 5 olguda (%29.5) enfeksiyon ve 2 olguda (%11.7) nonunion gelişti. Enfeksiyon ve nonunion gelişen olguların tümü internal fiksasyon uygulanan gruptan idi. YORUM: Aldığımız sonuçlarımız dirsek kondiler bölge yüksek enerjili ateşli silah yaralanması sonucu oluşan Tip III kırıklarda prognozun kötü olduğunu, kırık tedavisinde eksternal fiksasyonun daha az komplikasyon oranı taşıdığını gösterir niteliktedir.

Abstract

PURPOSE: Gustillo-Anderson Type III fractures of elbow condyles due to high energy gunshot wounds have high morbidity rate and are difficult to treat because of the anatomic structure of elbow, soft tissue problems, closeness with neurovascular structures, and because they are communited. The aim of this study is to present our treatment choice in acute term in this type of fractures. METHODS: Nineteen patients who were brought to the Department of Orthopedics and Traumatology, GATA between December 1993-December 1997 in the first 6-8 hours for high-energy gunshot wounds and were treated for Type III open elbow condylar area fracture are evaluated. Five were Type IIIa, 12 Type IIIb, and 2 Type IIIc. Urgent debridement, primary soft tissue closure and fracture stabilization were applied to all. Vascular repair was applied to cases with Type IIIc fractures. Internal fixation was applied to all Type IIIa cases and 5 of the Type IIIb cases. External fixation was applied to 7 Type IIIb cases and to all Type IIIc cases. RESULTS: Average follow-up period was 32 months (24.5-55 months) and 5 good (29.5%) and 12 bad (70.5 %) results according to Morrey criteria were observed. Forearm amputation was performed in 2 cases with vascular repair because of necrosis. In 5 cases infection (29.5%) and in 2 cases nonunion (11.7%) were observed during the follow-up period. These cases were from the internal fixation group. CONCLUSION: Our results show that there is poor prognoz in.Type III fractures that are results of high energy gunshot wounds in elbow condular area, and that external fixation has a lower rate of complication in the treatment of these type of fractures.
