Özet

AMAÇ: Bu çalışma ortopedik cerrahi girişimlerde, eldiven delinmeleri sonucu ortaya çıkan el yaralanmalarından korunmada, tek eldiven kullanımı yerine çift eldiven kullanımının etkisini belirlemek amacıyla yapılmıştır. YÖNTEM: 23.12.1994-6.10.1995 tarihleri arasında S.B. Haseki Hastanesi'nde gerçekleştirilen 101 Ortopedi ve Travmatoloji ameliyatı incelendi. Rasgele seçilmiş 44 ameliyat çift eldiven giydirilen ameliyat ekibiyle, 57 ameliyat ise tek eldiven giydirilen ameliyat ekibiyle gerçekleştirildi. Ameliyat sırasında eldiven delinmesi farkedilen eldivenlere ve ameliyat sonunda ameliyat ekibinin kullandığı tüm eldivenlere su doldurularak delik olup olmadığı test edildi. Eldivenlerdeki deliğin yeri ve sayısı yanında, eldiven delinmelerinin gruplara, ameliyat sürelerine ve cerrahi ekipte yer alan kişilerin konumuna göre dağılımları irdelendi, istatistiksel analizler Epilinfo 6.0 paket programı ile yapıldı. SONUÇLAR: Tek eldiven grubunda ameliyatların 42 (% 73.7)'sinde eldiven delinmesi saptanırken, çift eldiven grubunda hem dış hem de iç eldivenin delindiği ameliyat sayısı 8 (% 18) olarak saptandı. Tek eldiven ve çift eldiven gruplarında dış eldivenlerde delinme oranı (sırasıyla % 14.0, % 11.3) birbirine yakınken, çift eldiven grubunun iç eldivenlerinde delinme oranı % 2.8 olarak bulundu. Her iki grupta da ameliyatı yapan cerrahın eldiven delinmesi açısından eh yüksek risk altında olduğu belirlendi. YORUM :Ameliyatlarda çift eldiven kullanılmasının, eldiven delinmesi sonucu ortaya çıkan el yaralanmalarından ameliyat ekibini korumada tek eldivene göre daha etkili olmakla beraber kesin bir koruma aracı olmadığı düşünülmektedir.

Abstract

PURPOSE: This study was performed to determine the effect of double-gloving instead of standard singie-gloving in providing protection from hand injuries due to glove perforation during orthopaedic surgical interventions. METHODS: A total of 101 orthopaedic and traumatologic surgical interventions made in S.B.Haseki Hospital between 23.12.1994 and 6.10.1995 were evaluated. Randomly selected 44 operations were performed by double-gloved and 57 operations were performed by single-gloved operating team. Gloves which were perforated during the operation and all other gloves worn by the operating team during the operation were tested for perforations by water-filling. In addition to the site and number of the holes in the gloves, the distribution of glove perforations according to the groups, the durations of operations and the positions of the persons in the operation team were determined. The data were analyzed by Epilinfo 6.0 computer program. RESULTS: In the single - glove group, glove perforation was seen in 42 out of 57 operations (73.7 %), but in only 8 operations (18.0 %) in the double-glove group perforations were detected in both inner and outer gloves. The perforation rates of the outer gloves in the double glove group and the single glove group were similar (11.3 % and 14.0 %, respectively); however rate of perforationof the inner glove in the double glove group was only 2.8 %. The surgeon performing the operation was at the most risk of glove perforation both in the single-glove and double-glove groups. CONCLUSION: Double-gloving during the operations are significantly effective than single-gloving in protecting the operating team from hand injuries due to glove perforation but does not provide absolute protection.
