Özet

AMAÇ: Bu çalışmada amacımız, kronik hepatit B olgularında, HBsAg ve HBcAg'nin karaciğerdeki boyanma paterni ve boyanma şiddeti ile histolojik aktivite indeksi (HAI) arasındaki ilişkiyi değerlendirmektir. YÖNTEM: Kronik hepatit B tanısı alan 58 olguya ait karaciğer biyopsi örneği, çalışma grubu olarak seçildi. HAI'ne göre derecelendirilen olgulara ABC immünperoksidaz yöntemi ile HBsAg ve HbcAg uygulandı. Pozitif boyanan olgu sayısı ve boyanma şiddeti yüzde olarak değerlendirildi. SONUÇLAR: HAI düşük olan olgularda (minimal ve hafif kronik hepatit) HBsAg daha çok membranöz, HBcAg ise daha çok nükleer boyanma gösterdi. HAI yüksek olan olgularda ise (orta derecede ve şiddetli kronik hepatit) HBsAg'nin membranöz boyanması azalırken, HBcAg şiddetli sitoplazmik boyanma gösterdi. YORUM: Kronik B hepatitinde, HBcAg'nin nükleustan sitoplazmaya geçişi ile antijenik uyarıma neden olduğu ve T hücre bağımlı hepatosit yıkımına yol açtığı düşünülmektedir. Sonuçlarımız da bu görüşü desteklemektedir.

Abstract

PURPOSE: The purpose of the study was to evaluate the correlation of staining intensity and staining pattern of HBsAg and HBcAg with the histological activity index (HAI). METHODS: Liver biopsy samples taken from 58 patients with chronic hepatitis B were used for the study. HBsAg and HBcAg were applied by an ABC immunperoxidase method. Positive staining was determined by staining intensity in liver biopsies. RESULTS: HBsAg showed stronger membraneous staining, while HBcAg showed stronger nuclear staining in cases with low HAI (minimal and mild chronic hepatitis). In cases with high HAI (moderate and severe chronic hepatitis), membraneous staining intensity of HBsAg was reduced, while HBcAg showed stronger cytoplasmic staining. CONCLUSION: In chronic hepatitis B, cytoplasmic expression of HBcAg was suggested to cause antigenic stimulus for T lymphocyte, leading to hepatocyte damage. Our findings support this view.
