Nazal kavite ve paranazal sinüs kanserleri

Özet
AMAÇ: Sinonazal kanserler nadir görülen, spesifik olmayan belirtilerinden ve tutulan anatomik bölgelerin özelliklerinden dolayı tanısı gecikebilen hastalıklardır. Tedavi yöntemleri hakkında da tam bir görüş birliği yoktur. Nadir görülen bu tümörlerin tanı, tedavi ve prognozunu etkileyen faktörlerin araştırılması amaçlandı. YÖNTEM: Kliniğimizde 1991-1999 yılları arasında tedavi edilen, naza/ kavite ve paranazal sinüs tümörlü 33 olgu; yaş, cinsiyet, başvuru yakınması, tümörün primer yerleşim yeri ve evresi, histopatolojik tanı ve seçilen tedavi yaklaşımları açısından retrospektif olarak değerlendirildi. SONUÇLAR. Hastaların 20'si erkek, 13'ü kadın olup, yaşları 12 ile 70 yaş arasındaydı (ortalama 49.6). En sık görülen semptomlar tek taraflı burun tıkanıklığı (% 72) ve burun kanamasıydı (%13). Olguların büyük kısmı epidermoid karsinomdu (% 36.4). En sık primer yerleşim yeri maksiller sinüstü (%55). Maksiller sinüs tutulumu olan hastaların %72'si ileri evre (T3, T4) tümöre sahipti. Hastaların %30.3'üne sadece cerrahi, % 54.5'ine cerrahi+radyoterapi ve/veya kemoterapi ve % 15.2'sine sadece radyoterapi ve/veya kemoterapi uygulandı. 33 olgunun 787 (% 54.5) öldü, 15 olgu ortalama 29 ay hastalıksız olarak takip edildi. YORUM: Burun tıkanıklığı ve burun kanaması gibi spesifik olmayan semptomlarla başvuran özellikle ileri yaştaki hastalarda sinonazal tümör açısından ayrıntılı inceleme bu tümörlerin tanısındaki gecikme ve prognozun düzeltilmesinde yardımcı olabilir.
Abstract
Sinonasal tumors are seen rarely and can not be determined in early period because of their location and nonspecific symptoms. There is no consensus on which combination of management is the most effective. The aim of this study is to evaluate the diagnostic criteria, treatment modalities and prognosis of the sinonasal tumors. METHODS: The charts of 33 patients with malignant tumors of the nasal cavity and paranasal sinuses treated at our clinic between 1991-1999 were reviewed to document clinical presentation (age, gender, symptoms, primary site and stage of tumor, histopathological diagnosis), treatment and outcome. RESULTS: Twenty patients were males and 13 females. The mean age at the time of diagnosis was 49.6 years (range: 12-70). The most'frequent primary symptoms and signs were unilateral stenosis (72 %) and bleeding from the nose (13 %). The most common histopathologic diagnoses were epidermoid carcinoma. The site of the origin was the maxillary sinus in 55 % of the patients. Of 18 patients with maxillary sinus malignancies, 13 patients ( 72 %) presented with advanced tumors (T3,T4). The patients were treated as follows: 30.3 % with surgery alone, 54.5 % surgery and radiotherapy and/or chemotherapy, and 15.2 % with radiotherapy and/or chemotherapy. Of the patients, 18 were died and 15 remained no evidence of disease. The follow-up time of surviving patients ranged from 17 months to 5 years (mean 29 months). CONCLUSION: The curative treatment of sinonasal tumors depends on the tumor stage and surgical resectability. In patients (especially older patients) with nonspecific symptoms as unilateral stenosis and nasal bleeding careful examination and usage of advanced imagjng technics may allow for earlier sinonasal m'alignant tumor discovery and improve prognosis.
