Çocuklarda tonsillektomi sonrası görülen bulantı ve kusmada ondansetron, deksametazon ve droperidol'ün etkinliğinin karşılaştırılması

Özet
AMAÇ: Çalışmamızda, azot protoksitle kombine intravenöz anestezi uygulanan günübirlik tonsillektomi vakalarında, ondansetron, deksametazon ve droperidolün intraoperatif hemodinamik değişikliklerde, postoperatif kusma ve derlenme üzerine etkilerinin karşılaştırılması amaçlandı. YÖNTEM: Yaşları 3-10 arasında değişen, ASA I-II, elektif tonsillektomi uygulanacak 60 çocuk hasta çalışma kapsamına alındı. Anestezi öncesi katı oral gıda alımı operasyondan önce sekiz saat, sıvı gıda alımı ise dört saat önce yasaklandı. Olguların hiçbirisine premedikasyon uygulanmadı. Hastalarda elektrokardiyogram, sistolik ve diyastolik kan basınçları ölçümü ve SpO2 ile standart monitörizasyon uygulandı. Anestezi indüksiyonu 2.5-3 mg.kg-1 propofol ile sağlandı. Kas gevşetici olarak 0.6 mg-1.kg-1 rokuronyum uygulandı. Entübasyondan 3 dk sonra Grup I' de 150 $mu$g.kg-1 ondansetron (n=15), Grup II' ye 150 $mu$g-1.kg-1 deksametazon (n=15), Grup III' e 75 $mu$g.kg-1 droperidol (DHB) (n=15), Grup IV' e 2 ml serum fizyolojik (n=15) uygulandı.Anestezi idamesi N2O ile birlikte propofol-alfentanil infüzyonu ile sürdürüldü. Perioperatif hemodinamik değişiklikler, postoperatif uyanma ve ekstübasyon zamanı, taburcu olma süresi, kusma atakları kaydedildi.
Abstract
PURPOSE: The aim of the study was to compare the effects of ondansetron, dexamethasone and droperidol on perioperative hemodynamic changes, and postoperative vomiting and recovery. METHODS: Sixty patients between 3-10 years of age underwent elective tonsillectomy. ASA status I or II were included in the study. Oral foods were prohibited eight hours and liquids four hours before the operation. None of the patients received premedication. Standard monitorization included electrocardiography, non-invarsive blood pressure and Sp02 . Anesthesia was inducted by propofol 2.5-3 mg.kg-1. Neuromuscular block was provided by rocuronium 0.6 mg.kg-1. Three minutes after endotracheal intubation, Group I received 150 $mu$g.kg-1 ondansetron (n=15), Group II received 150 $mu$g.kg-1 dexamethasone (n=15), and Group III received 75 $mu$g.kg-1 droperidol (n=15). Group IV received 2 ml saline (n=15). Anesthesia was maintained by propofol-alfentanil infusion with N2O. The perioperative hemodynamic changes, and postoperative awakening/ extubation time, time to discharge from hospital and vomiting attacks were recorded. RESULTS: The demographic findings were similar (p>0.05). No differences were found in hemodynamic changes of the groups when compared at different times (p>0.05). The incidence of postoperative total vomiting was similar in all groups (p>0.05). No differences were found in duration of anesthesia, extubation time and recovery time between the groups. CONCLUSION: In patients undergoing tonsillectomy with intravenous anesthesia carried out by N2O and propofol-alfentanil combination, ondansetron, dexamethasone and droperidol applied after induction have similar effects on hemodynamic changes and postoperative recovery to each other in preventing postoperative vomiting.
