Özet

Bu çalışmada 1992-1996 yılları arasında hastanemizde endoskopi yapılan çocuklarda üst gastrointestinal kanama nedenlerini restrospektif olarak araştırılmıştır. Endoskopi yapılan 974 hastadan 97'sinde bize başvuru nedeni kanama idi. Bu vakaların yaş ortalaması 7.4 yıl olup, kız/erkek oranı 42/55 idi. Altısı dışında tümüne ilk 24-48 saat içinde endoskopi yapılmıştı. Hastaların 42'si (% 43) sadece hematemez, 29'u (% 30) sadece melena, geri kalanları ise (% 27) hematemez ve melena yakınması ile getirilmişti. Yapılan endoskopi sonucunda 16'sında (%16.5) ösefagus varisi, 32'sinde (%33) ösefajit, 67'sinde (% 69) gastrit (44'ünde antral nodüler görünüm vardı), 28'inde (% 29) duodenit saptandı. Beşi ösefagusda, ikisi midede preplorik bölgede, dokuzu duodenumda olmak üzere toplam 15 hastada 16 ülser görüldü. Bir vakada hem ösefagus, hem de preplorik bölgede ülser vardı. Altı vakada ise endoskopik patoloji saptanmadı. Helicobacter pylori enfeksiyonu araştırılan 80 hastanın 43'ünde (% 54) H. pylori gastriti saptandı. Yirmidokuz hastanın (% 30) öyküsünde ise aspirin alımı vardı. Vakalarımızın kanama nedenleri literatür verileri ile uyumluluk gösterirken, aspirin öyküsünün % 30 yer tutması dikkat çekici bulundu.

Abstract

The cause of upper gastrointestinal bleeding varies in different age gorups. In newborns, swallowed maternal blood is the most common cause of upper gastrointestinal bleeding. In the following years, gastritis, esophagitis, esophageal varices and duodenal ulcers become the more common causes of upper gastrointestinal bleeding. We have restrospectively investigated the causes of upper gastrointestinal bleeding among patients who underwent endoscopie examination between 1992-1996. Among 974 patients investigated by endoscopy, 97 were admitted to hospital with bleeding. The mean age of the study population was 9.9±3.7 years (43% female). All except six patients had endoscopy in the first 24-48 hours. Forty-two cases (43%) presented only with the complaint of hematemesis, 29 (30%) only with melena, and the remainder with hematemesis and melena. During endoscopy, esophageal varices were identified in 16 patients (16.5%), esophagitis in 32 patients (33%), gastritis in 67 patients (69%), antral nodules in 44 patients (45%) and duodenitis in 28 patients (29%). Sixteen ulcers were identified in 15 patients: five in the esophagus,two in the antral prepyloric region and nine in the duodenum. In one case, there was an ulcer both in the esophagus and in the prepyloric region. Six cases were evaluated as normal endoscopically. Among 80 cases investigated for Helicobacter pylori, only 43 cases were positive (54%). In 29 cases (30%) there was aspirin intake before bleeding. The results of our study were similar to the results of other published studies.
