Vena kava inferior ve sağ kalpte trombüse neden olan atipik konjenital mezoblastik nefroma vakası

Özet
Mezoblastik nefroma genellikle ilk altı ayda karşılaşılan prognozu son derece iyi, konjenital bir böbrek tümörüdür. Ancak daha büyük bebeklerde agresif bir gidiş gözlenebilir. Biz burada Wilms tümörü gibi seyir gösteren vena kava inferior ve sağ kalpte trombüse neden olan bir atipik mezoblastik nefroma vakas; seyrek görülmesi nedeniyle sunmayı uygun bulduk. İki yaşında iken atipik mezoblastik nefroma tanısı alan hasta, cerrahi girişimden sonra altı ay süre ile kemoterapi aldı. Ancak iki yıl sonra rekürrens saptanarak cerrahi girişim ve kemoterapi uygulandı. Kemoterapinin sonlandırılmasmdan sonraki iki aylık süre içinde abdominal tomografi ve Doppler ultrasonografi ile vena kava inferior ve sağ kalpte trombus saptandı. Açık kalp cerrahisi ile sağ kalpteki tümör trombüsü temizlendi ve vena kava inferior'e ligasyon uygulandı. Daha sonra karboplatin ve VP-16'dan oluşan kemoterapi verilen hasta herhangi bir komplikasyon olmaksızın on aydan beri izlenmektedir.
Abstract
Congenital mesoblastic nephroma is the most common renal tumor in early infancy with a generally excellent prognosis, but it may behave aggressively in older infants. In this paper, we present a case of atypic congenital mesoblastic nephroma (ACMN) which progressed as Wilms' tumor with a tumor thrombus filling the vena cava inferior (VCI) and right atrium. A two-year-old boy was diagnosed as ACMN and was treated with a right uretero nephrectomy and chemotherapy. Tumor recurrence was observed two years after diagnosis and the tumor was completely resected; postoperative chemotherapy was considered necessary. A thrombus filling the VCI and the right atrium was observed within two months after chemotherapy by computerized tomography. Two tumor thrombi within the right atrium were resected and the VCI was ligated by cardiopulmonary bypass. The case was treated postoperatively with VP-16 and carboplatin. The case has been followed for 10 months without any complication.
