Özet

Amaç: Kısa segment Hirschsprung hastalığının (KSHH) tedavisinde myektomi/sfinkteromyektomi (M/S) modifikas- yonları kullanılmaktadır. Bu çalışmada KSHH’da M/S ya- pılan olgular, demografik, klinik ve M/S sonrası sonuçlar ve takip açısından analiz edildi. Hastalar ve Yöntem: 1997’den 2009’a kadar, 20 olguya KSHH tanısıyla M/S yapıldı. Olgular hikâye, fizik muayene bulguları, preoperatif lavman opaklı kolon grafisi, anorek- tal manometri, rektal biyopsi sonuçları ve postoperatif ta- kip açısından geriye dönük olarak tarandı. Bulgular: Çalışma grubu 13 erkek 7 kız olup, yaş ortala- ması 36 ay (1ay-10 yaş) idi. En sık başvuru şikâyeti kro- nik konstipasyondu (% 75). Bunu sırasıyla, karın şişliği ve kusma (% 20) ve kötü kokulu ishal (% 5) takip etmekteydi. Hikâyede gecikmiş mekonyum pasajı (>48 saat) 8 (% 47) olguda mevcuttu. Fizik muayenede olguların 9’unda (% 45) rektal veya karında fekalom, 3’ünde (% 15) karında şişlik ve 3’ünde (% 15) rektal tuşe sonrası fışkırır tarzda gaz/gayta çıkışı saptandı. Diğer 5 olgunun fizik muayenesi normaldi. Lavman opaklı kolon grafisinde 19 olguda rektal dilatasyon gözlendi. Bu olguların 10’unda (% 53) rektumda transizyo- nel zon mevcuttu. Baryum retansiyonu ise 17 (% 85) olguda saptandı. Anorektal manometri yapılan 17 olgunun 15’inde (% 89) rektoanal inhibitör refleks alınamazken, 3’ünde (% 17) anal hipertoni de mevcuttu. Hirschsprung hastalığı tanısı 10 olguda (% 50) myektomi öncesi rektal biyopsi ile konurken, diğer olgularda frozen section ile peroperatuar konuldu. Anal hipertoni saptanan 3 olguya sfinkteromyek- tomi diğer tüm olgulara sadece myektomi yapıldı. Rektal myektomide çıkarılan kas şeridinin uzunluğu 2-8 cm ara- sında (ort.: 4,1 cm) değişmekteydi. Kas şeridinin distal ucu tüm olgularda aganglionik iken, proksimal uç 13 (% 65) ol- guda ganglionik, 7 olguda ise aganglionikti. Aganglionozis saptanan olgulardan ikisine definitif cerrahi uygulanırken diğerleri takibe alındı. Postoperatif takip süresi ortalama 17 ay olup, 9 olguda ortalama 3 ay süren laksatif ihtiyacı oldu. Diğer olguların takibi problemsizdi. Sonuç: Kısa segment Hirschsprung hastalığında myektomi etkili ve az invaziv bir tedavi yöntemidir. Anal hipertoninin eşlik ettiği olgularda sfinkteromyektomi tercih edilmelidir. Myektomide çıkarılan kas şeridinin proksimali aganglionik gelse bile definitif cerrahi klinik semptomları düzelmeyen olgularda planlanmalıdır.

Abstract

Aim: Modification of myectomy/sphincteromyectomy co- uld be useful in the surgical treatment for short-segment Hirschsprung’s disease (SSHD). This study analyzed the outcome of M/S in SSHD for demographic, clinical findings and results and follow-up after M/S. Patients and Methods: From 1997 to 2009, 20 children were treated with M/S for SSHD. They were retrospectively screened on the basis of history, examination results, pre- operative barium enema (BE) study, anorectal manometry and rectal biopsy findings and postop follow-up. Results: The study group included 13 boys and 7 girls with a mean age of 36 months (1 month to 10 years). The most common symptom was chronic constipation witho- ut soiling (75 %). Others were abdominal distension and vomiting (20 %) and enterocolitis (5 %). Delayed meconi- um passage (>48 hours) was found in 8 (47 %) patients. Physical examination findings; rectal and/or abdominal fecaloma in 9 (45 %) patients, abdominal distention in 4 (20 %) patients, passage of gas and feces after rectal examination in 3 (15 %) patients. The remaining patients had normal physical examination. Preoperative BE sho- wed rectal dilatation in 19 cases, while narrow segment was only detected in 10 patients (53 %) with low cone position. Barium retention, more than 24 hours, was found in 17 (85 %) patients. An anorectal manometry revealed lack of rectoanal inhibitory reflex in 15 (89 %) patients. Anal hypertonia was only found in 3 (17 %) patients. The diagnosis of SSHD was proven histopathologically (preoperative rectal biopsy in 10 (50 %) patients, others perioperative frozen section biopsy). A simple myectomy was performed in all patients except 3 cases who had anal hypertonia. In these cases were treated by sphincterom- yectomy. The length of the rectal muscle strip ranged from 2 to 8 cm (mean: 4.1 cm). Distal end of the strips revealed aganglionosis in all cases but, normal ganglia were seen at the proximal end of the strip only in 13 (65 %) cases. Two of the remaining patients were treated with definitive surgery. Others underwent clinical observation. Average follow-up was 17 months. Laxative treatment was only re- quired in 9 patients during 3 months. Remaining patients are symptom-free. Conclusion: Myectomy is an effective and minimal invasi- ve treatment for SSHD. If the patient has anal hypertonia, sphincteromyectomy can be performed. If the proximal end of the strip is aganglionic, definitive surgery can be delayed until persistent clinical findings develop.
