Two staged gastrectomy for the treatment of gastric tumor perforations

Abstract

The treatment of three cases of perforation secondary to gastric tumor was presented. If feasible primary gastrecto¬my was considered the treatment of choice, when the pa¬tients condition precludes this operation a delayed gas¬trectomy which follows 2 to 3 weeks after omental patch repair of the perforation^was recommended in the litera¬ture. We performed primary gastrectomy in our first pa¬tient whose general condition was good and perforation was limited by omentum. The general condition of the re¬maining two patients were poor and having generalized peritonitis, went to two stage operations. In these two pa¬tients, within 48 hours of the primary repair of the perfo¬rations, early reoperations were carried out and total gas-trectomies were done. Early reoperation and gastrectomy after a primary repair of the gastric perforation is more advantageous for the patients with resectable tumors whom their condition precludes primary gastrectomy in their first operation.
